Perioperative und traumabedingte Blutungen stellen nach wie vor eine der häufigsten Ursachen für Morbidität und Mortalität in der Akutmedizin dar. Eine rasche und präzise Gerinnungsdiagnostik ist entscheidend, um eine adäquate Hämotherapie einzuleiten und den Verbrauch von allogenen Blutprodukten zu minimieren.
Viskoelastische Testverfahren wie ClotPro®, Multiclot®, Quantra®, ROTEM Sigma®, und TEG 6s® stehen hierbei als wesentliche Instrumente im Point-of-Care(POC)-Setting zur Verfügung. Sie ermöglichen eine dynamische Beurteilung der Gerinnungseigenschaften in Echtzeit und liefern damit eine funktionelle Einschätzung der Hämostase, die über klassische Laborparameter hinausgeht.
In dieser Übersichtsarbeit werden die technologischen Grundlagen, die aktuellen Gerätegenerationen sowie die klinische Evidenzlage der viskoelastischen Testverfahren dargestellt. Darüber hinaus werden gerätespezifische Behandlungsalgorithmen und praxisrelevante Strategien in verschiedenen klinischen Kontexten (Herzchirurgie, Traumatologie, Geburtshilfe, Lebertransplantation und Intensivmedizin) diskutiert.
Abschließend werden Kosteneffizienz, Perspektiven zukünftiger Entwicklungen und die Integration künstlicher Intelligenz in die POC-Diagnostik beleuchtet.
Perioperative and trauma-related hemorrhages still constitute one of the most frequent causes of morbidity and mortality in acute medicine. Quick and precise coagulation diagnostics are decisive for the induction of adequate hemotherapy and minimizing the application of allogenic blood products.
Viscoelastic test methods such as ClotPro®, Multiclot®, Quantra®, ROTEM Sigma®, and TEG 6s® are available essential instruments applicable in a point-of-care (POC) setting. They enable dynamic coagulation assessment in realtime and thus a functional evaluation of hemostasis which exceeds classical laboratory parameters.
In this review article we outline the technological foundations, the current generations of appliances as well as the clinical evidence pertaining to the viscoelastic test procedures applied. In addition, device-specific treatment algorithms and practically relevant strategies are discussed in various contexts of clinical work (cardiac surgery, traumatology, obstetrics, liver transplantation and intensive medical care).
Finally, we discuss the cost effectiveness, the perspectives of future developments, and the integration of artifical intelligence into POC diagnostics.
