Intravenous regional anaesthesia - a new approach in clinical application

Zusammenfassung: Am Beginn unserer Bemühungen stand, die Dosis des Lokalanästhetikums zur Durchführung einer intravenösen Regionalanästhesie für operative Eingriffe am Arm zu reduzieren. So haben wir im ersten Untersuchungskomplex bei 80 Patienten der Risikogruppen I und II 100 mg Lidocain mit 0,05 mg Fentanyl und 0,5 mg Pancuroniumbromid kombiniert.


Ziel war es,die Effektivität dieser Kombination hinsichtlich der Analgesie und Relaxation während operativer Eingriffe sowie ihre Sicherheit nach sofortigem Ablassen des Manschettendruckes zu untersuchen. Die Patienten wurden randomisiert folgenden Gruppen zugeordnet: Gruppe A (n=15) erhielt 100 mg Lidocain verdünnt in 40 ml NaCl i.v.; Gruppe B (n=15), Gruppe C (n=15) und Gruppe D (n=15) erhielten zusätzlich zu Lidocain entweder 0,05 mg Fentanyl oder 0,5 mg Pancuroniumbromid oder beide Medikamente. In Gruppe E (n=20) wurde der Tourniquet sofort nach i.v.-Injektion von 100 mg Lidocain, 0,05 mg Fentanyl und 0,5 mg Pancuroniumbromid abgelassen. Die Auswertung der Ergebnisse fiel deutlich zugunsten der Gruppe D aus. Die Patienten bewerteten die Analgesie als gut oder ausgezeichnet. Der Zusatz von 0,05 mg Fentanyl in Gruppe B erhöhte die Qualität der Analgesie signifikant verglichen mit Gruppe A,wobei der Vorteil der niedrigen Dosierung des Lokalanästhe-tikums erhalten blieb. Als zusätzlicher Vorteil des Verfahrens stellte sich in unserer Studie die komplette Muskelrelaxation heraus. Sie war durch den Zusatz von Pancuroniumbromid in den Gruppen C und D verglichen mit Gruppe A und B ausgeprägt.Die hämo-dynamischen und respiratorischen Parameter stiegen in Gruppe A, B und C während des Operationsverlaufs signifikant an, während sie in Gruppe D als Ausdruck suffizienter Analgesie stabil blieben. In Gruppe E klagten einige Probanden nach sofortigem Ablassen des Tourniquets über leichten Schwindel und vorübergehende Sehstörungen. In einem Fall kam es zu Erbrechen und Blutdruckabfall mit einem systoli-schen Blutdruck von 90 mmHg fünf Minuten nach Ablassen des Tourniquets. Nachdem diese Ergebnisse eine gute Wirksamkeit der Kombination von Lidocain mit Fentanyl und Pancuroniumbromid demonstrierten, auf der anderen Seite jedoch ein nicht unerheblicher Prozentsatz an Patienten der Gruppe D einen geringen Schmerz ver-spürten, kam es uns vor allem auf die weitere Erhöhung des Prozentsatzes der mit dem neuen Konzept der intravenösen Lokalanästhesie vollständig - zufriedenen Patienten an. So wurde eine Dosis von 150 mg Lidocain in Kombination mit 0,5 mg Pancuroniumbromid und 0,05 mg Fentanyl verwendet. Unter diesem Behandlungs-- regime konnte zum einen im Bereich der Weichteilchirurgie eine nahezu 100%ige Zufriedenheit verzeichnet werden,zum anderen zeigten sich,verglichen mit der Gruppe E, keine gravierenderen Nebenwirkungen. Unter besonderer Beachtung der immerhin weit verbreiteten Annahme, daß Prilocain einen Benefit zur Durchführung der intravenösen Regionalanästhesie haben könnte, untersuchten wir zwei weitere Gruppen mit einer Dosierung von 150 mg und 200 mg Prilocain im Rahmen der Weichteil-- chirurgie. Dabei zeigte sich zunächst, daß Prilocain keinen Vorteil gegenüber dem Lidocain aufweist, wei-terhin,daß die Dosiserhöhung keinen weiteren Vorteil brachte.Prilocain erwies sich als dem Lidocain durchaus ebenbürtig, wenngleich nicht überlegen. Nur bei - knochenchirurgischen Eingriffen kam es zu partiellen Versagern, so daß wir ebenfalls eine Gruppe von Patienten mit einer höheren Dosis von 250 mg Lidocain bei unverändertem Zusatz von Fentanyl und Pancuroniumbromid ebenfalls mit sehr gutem Erfolg - untersuchten. Diese Kombinationstechnik stellt bei ausreichender - Analgesie und Muskelrelaxation sowohl bei krankengymnastischen Übungsbehandlungen als auch im Bereich der Weichteilchirurgie und insbesondere bei orthopädisch-traumatologischen Operationen eine wertvolle Bereicherung der intravenösen Regional-anästhesie und somit der anästhesiologischen - Möglichkeiten dar. Unter Verwendung sehr geringer Dosen von Lokalanästhetika - wie im ersten Teil unserer Untersuchungen - muß unbedingt auf eine absolute Blutleere Wert gelegt werden.

Summary Summary: In an attempt to reduce the dose of local anaesthetic during intravenous regional anaesthesia of the upper limb, we combined 100 mg lidocaine with 0,05 mg fentanyl and 0.5 mg pancuroniumbromid.The study was designed in a randomized fashion to deter-mine the efficacy during surgery and to evaluate its safety after immediate deflation of the tourniquet fol-lowing i.v.drug injection.In the first investigation com-plex,80 patients ASA physical status I or II were assig-ned to the following groups:Group A (n=15) received 100 mg lidocaine diluted in 40 ml NaCl i.v.;Groups BD (n=15 in each group) received 100 mg of lidocaine diluted in 40 ml NaCl,with addition of 0.05 mg of fen-tanyl (group B) or 0.5 mg of pancuroniumbromide (group C), or both (group D) to a total volume in all groups of 40 ml. Patients in groups A-D underwent elective operations on the forearm,wrist,and hand;for evaluation of safety, in 20 volunteers (group E) the tourniquet was immediately released after IV injec-tion of the three drugs at the previously described doses. The analgesic effect was more profound in group D compared with groups A-C. In group D, 9 of 15 patients had excellent analgesia. In 6 patients, pain was experienced at the beginning of surgery, but 5 minutes thereafter patients remained pain free. The addition of 0.05 mg fentanyl in group B increased the quality of the analgesia significantly compared with group A, however analgesia in group B was obtained successfully in only 26.6% of cases compared with group A with 13.3%. Skeletal muscle relaxation was profound by the addition of pancuroniumbromide in the groups C and D compared with group A and B. The hemodynamic and respiratory parameters rose in group A, B and C during surgery significantly, while they remained stable in group D as a sign of sufficient analgesia.Signs indicating toxicity were not encounte-red after release of the tourniquets in these groups.In group E, the volunteers complained of minor events, such as mild dizziness and transient visual disturban-ces. In one case, vomiting and hypotension with a systolic arterial blood pressure of 90 mm Hg occurred 5 minutes after release of the Tourniquet. Although the results of this approach to IV regional anaesthesia with the combination of lidocaine, fen-tanyl and pancuroniumbromide demonstrated ade-quate analgesia and relaxation during upper limb sur-gery, some patients in group D still felt a small pain particularly at the beginning of surgery. So we tried in a second investigation complex to further increase the number of patients satisfied with this new approach in any case of upper limb surgery.Thus,a dose of 150 mg lidocaine in combination with 0,5 mg pancuronium-bromide and 0.05 mg fentanyl was used. Under this handling regime all patients undergoing soft part sur-gery were completely satisfied and more serious side effects with the increased dose of lidocaine were not observed as compared with group E.Special consider-ation was also paid to the wide-spread acceptance that prilocaine could have a benefit for intravenous regional anaesthesia. We therefore examined two further groups with a dosage of 150 mg and 200 mg prilocaine in patients undergoing soft part surgery on the upper limb.The results showed that prilocaine does not indi-cate an advantage compared to lidocaine and that the dose increase from 150 mg to 200 mg prilocaine did not bring any further effect in intravenous regional anaesthesia. Prilocaine proved to be quite equal to lidocaine. Only with bone-surgical interventions were there partial failures, so that we likewise examined another group of patients with a higher dose of 250 mg lidocaine in combination with fentanyl and pancuroni-umbromide as already described. This drug combination technique for intravenous regional anaesthesia represents a valuable enrichment of the anaesthetic possibilities with sufficient analge-sia, muscle relaxation and safety for patients under-going upper limb surgery in particular orthopaedic operations as well as for patients needing gymnastic exercise handlings for pain relief. Using very small doses of local anaesthetics as in the first complex of our investigations importance must be attached to an absolute blood emptiness for upper limb surgery.
