Zusammenfassung

Zusammenfassung: Neben der klinisch sinnvollen Kombination von Wirkstoffen haben in den letzten Jahren Arzneistoff-Interaktionen (AI) wiederholt die Pharmakotherapie kompliziert, deren Sicherheit ein-geschränkt und zum Rückzug von Pharmaka aus dem Markt geführt.


AI können sowohl innerhalb als auch außerhalb des menschlichen Organismus auftreten.Prinzipiell lassen sich AI in in vitro (Inkompatibilitäten), pharmakoki-netische und pharmakodynamische Wechselwirkungen unterscheiden.Allerdings sind die Kenntnisse über diese Zusammenhänge im einzelnen teilweise noch lückenhaft.AI sind aufgrund der Polymedikation bei hospitalisierten Patienten häufig und oft mit uner-wünschten Nebenwirkungen verbunden. Im Rahmen einer Anästhesie kommen zahlreiche potente Wirkstoffe mit relativ geringer therapeutischer Breite zur Anwendung. Das Risiko von AI ist deshalb in der Anästhesie sehr groß. Viele der unerwünschten AI können die vitalen Schutzreflexe beeinträchtigen und somit ohne engmaschige Überwachung zu teilweise schwerwiegenden Komplikationen führen. Diese Übersicht beschäftigt sich mit den grundlegenden Mechanismen der verschiedenen Wechselwir-kungen.Dabei werden sämtliche Schritte zwischen der Applikation des Wirkstoffs und dem Auftreten des klinischen Effektes (Resorption, Verteilung/Protein-bindung, Biotransformation und Exkretion) einer Diskussion unterzogen.Der wichtigste Entstehungsort für anästhesiologisch bedeutsame pharmakokinetische AI ist das mikrosomale Cytochrom P-450-System der Leber. Des weiteren werden pharmakodynamisch bedingte AI diskutiert. Über aufgetretene mögliche AI während anästhesiolo-gischer Maßnahmen wird zu selten berichtet, insbesondere über AI, die von geringfügiger Natur sind (z.B. Flush). Daher ist die Anzahl von AI im Rahmen der Anästhesie unbekannt.Es wird eine Checkliste der möglichen AI vorgestellt, die für die Anästhesie relevant sein könnten.

Summary Summary: In recent years undesirable drug interac-tions have complicated the safety of drug therapy, repeatedly leading to the withdrawal of drugs from the market. Interaction can occur both within and outside the body. It can be divided into in vitro (incompatibility), pharmacokinetic and pharmacodynamic interactions. However, the knowledge regarding these connections is still incomplete. Drug interactions are frequent in hospitalized patients because of the polymedication under this situation. They are often combined with unwanted side effects. During anaesthesia, numerous potent drugs with relatively small therapeutic width are used, and therefore the risk of interaction is very high. The vital protective reflexes may be influenced by unwanted drug interactions,leading to severe com-plications. This review is focused on the basic mechanisms of interaction. All steps between the application of the drug and the occurrence of the clinical effect (resorption, distribution/binding to proteins, biotransformati-on, excretion) are discussed. The most important ori-gin of DI with anaesthesiological relevance is the microsomal cytochrome P-450 system localized within the liver. Unfortunately, anaesthetists tend not to report inter-actions – especially those of a minor nature such as flushing – and hence the true number of drug interac-tions is unknown. We include a check list indicating the nature of important drug interactions that the anaesthetist may encounter.
