Zusammenfassung: Der "SPILKER"-Block (Selektive Peronäus-Infiltration mit latero-kranialer Einstich-Richtung) wird als modifizierte Form der Peronäusblockade im Vergleich zum Ischiadicusblock vorgestellt. Die Wirkung wurde retrospektiv anhand einer klinischen Studie untersucht,in der 30 Patienten mit einem "SPILKER"-Block und 30 Patienten mit einem Ischiadicusblock behandelt wurden.
Mittels einer retrospektiven Datenanalyse wurde die Anal-gesiestärke (anhand der Numerischen Analogskala = NAS) und Analgesiedauer (in Stunden) verglichen. Hinsichtlich der Analgesiestärke und der Analgesie-dauer ist der "SPILKER"-Block im Vergleich zum Ischiadicusblock als Alternative bei bestimmten Pati-entengruppen zu betrachten. Vorgehensweise, Wirk-mechanismus und Indikationen werden vorgestellt. Die Inzidenz von direkter Nervenschädigung durch diese alternative Methode könnte dabei geringer sein.

Summary: The present study introduces the so-called "SPILKER" block (abbreviation of a German term denoting a selective peroneal infiltration in which the puncture is made in a lateral cranial direction) as a modification of the peroneal nerve block in compari-son to the sciatic nerve block. The effects of these methods were investigated in a clinical trial, in which 30 patients received a "SPILKER" block and other 30 patients a sciatic nerve block. By means of retrospec-tive data analysis, the pain score (numerical analogue scale) and analgesic duration (hours) were compared. No significant difference in pain score reduction and analgesic duration was found.The "SPILKER" block may therefore be considered as an alternative to the sciatic nerve block in certain groups of patients. Procedures, modes of action and indications are pre-sented. The incidence of direct nerve injury may be lower if a "SPILKER" block is used than if a conven-tional peroneal or sciatic nerve block is employed.
