Schwere Sepsis bei Peritonitis im Rahmen einer Malaria-tropica-Infektion

Zusammenfassung: Ein zwanzigjähriger Patient mit einer schweren Sepsis bei Peritonitis und einem Parasitenbefall von 49% der Erythrozyten im Rahmen einer Malaria tropica kam auf einer operativen Intensivstation zur Aufnahme. Die mit der Malariainfektion einhergehenden Mikrozirkulationsstörungen in Lunge und Leber imponierten bei dem Patienten in Form von Mikroinfarkten, welche mittels Computertomographie dargestellt wurden.


Im Abdomen manifestierten sie sich als nicht-okklusiver Mesenterialinfarkt mit Durchwanderungsperitonitis. Mikrozirkulationsstörungen am Herzen imponierten in Form einer akut aufgetretenen, deutlich reduzierten linksventrikulären Pumpfunktion, die sowohl klinisch als auch mit Hilfe der transösophagealen Echokardiographie nachvollzogen werden konnte. Mit der Bestimmung von Procalcitonin und Interleukin-6 neben anderen Infektionsparametern verfügten wir über einen Parameter, mit dessen Hilfe es möglich war; Informationen über den Verlauf der inflammatorischen Reaktion zu gewinnen und unseren antiinfektiösen Therapieerfolg zu beurteilen. Durch eine intensivmedizinische Behandlung der eingetretenen Organdysfunktionen und eine frühe chirurgische Therapie in Kombination mit einer effektiven antiinfektiösen Therapie konnte der Patient trotz einer statistisch belegten hohen Mortalitätsrate erfolgreich behandelt werden.

Summary Summary: A 20-year-old male patient suffering from a complicated Plasmodium falciparum malaria infection and severe sepsis caused by peritonitis was admitted to a surgical intensive care unit. On admission, parasitaemia was 49%. Detection of focal areas of microinfarction in the lung and liver using computed tomography demonstrated characteristic malaria-associated disturbances of microcirculation. In the abdomen, impairment of microcirculation was induced by non-occlusive mesenteric infarction accompanied by diffuse peritonitis. An acute reduction in the left ventricular ejection fraction was detected by transoeso-phageal echocardiography and attributed to disturbances of myocardial microcirculation. Monitoring of the systemic inflammatory reaction by assessment of interleukin- 6 and procalcitonin plasma levels indicated a fast response to antibiotic treatment and a reduction in the overall inflammatory activity. Early surgical intervention as well as adequate antibiotic therapy combined with effective fluid resuscitation was the basis of the successful treatment of the multiple organ dysfunction and life-threatening complicated falciparum malaria infection of the patient, notwithstanding the statistically confirmed high mortality rate.
