Zusammenfassung: Personalkosten stellen mit über 70% den größten Kostenfaktor in Krankenhäusern dar. Entsprechend schwierig sind für leitende Anästhesisten die Rahmenbedingungen in Verhandlungen über Stellenpläne und der Nachweis des tatsächlichen Personalbedarfes.


Dies wird durch die Tatsache verstärkt, dass mit Einführung der Diagnosis-Related-Groups (DRG) ein grundlegender Paradigmenwechsel in der Bewertung von Krankenhausleistungen stattgefunden hat. In diesem Beitrag wird unter Berücksichtigung der historischen Entwicklung der Personalbedarfskalkulation in der Anästhesie ein neues, den Bedingungen im DRG-System angepasstes Personalbedarfsmodell vorgestellt. Die Umsetzung dieses Modells in die Praxis wird erleichtert durch ein neu entwickeltes Kalkulationsinstrument, das jedem Kollegen die Kalkulation des eigenen Personalbedarfes auf der Basis der eigenen Betriebsorganisation erlaubt.

Summary Summary: Accounting for more than 70%, personnel costs represent the largest cost factor in the hospital. This fact makes the general conditions for negotiations on staff planning and the demonstration of the actual personnel requirements all the more difficult for anaesthetists with managerial responsibility. This problem situation has been further aggravated by the introduction of the diagnosis-related groups (DRGs), which represents a basic paradigm change in the evaluation of hospital services. The present article presents a new personnel requirement model that is oriented to the conditions of the DRG system and which also takes account of the historical development of the calculation of staff needs in anaesthesia. The implementation of this model in practice is facilitated by a newly developed calculation instrument that permits the calculation by the individual department of the specific staff requirements on the basis of its own particular organisation.
