Zusammenfassung

Fragestellung: Für die Ernährung von Intensiv­pati­enten wurde die intensivierte Insulintherapie (IIT) 2001 von van den Berghe et al. erstmals anhand eines Kollektivs septischer Patienten beschrieben. Nachdem diese Arbeit eine signifikante Reduktion der Patientensterblichkeit hatte zeigen können, erbrachten weitere Studien (z.B.

VISEP-Studie) un­ein­heitliche Ergebnisse. Ziel dieser Studie ist die Unter­suchung des Einflusses unterschiedlicher Glukose- und Insulinkonzentrationen auf die Viabilität von Endothelzellen in vitro. Methodik: Endothelzellen (HUVEC = human umbilican vein endothelial cells) wurden in Versuchs­an­sätzen mit Insulin (0 sowie 0,005 und 0,01 iE/ml] und Glukose [0 sowie 5 mmol/ml (~90 mg/dl) und 10 mmol/ml (~180 mg/dl)) inkubiert. Im Durchfluss­zyto­meter wurden Nekrose (Propidiumjodid) und Apop­tose (Annexin) der Zellen gemessen. Ergebniss: Die Untersuchungen zeigen, dass in dem gewählten Zellmodell bei Einsatz verschiedener Insulin- und Glukosekonzentrationen kein signifikanter Unterschied für die gemessene Rate an Apop­tose/Nekrose der Endothelzellen nachzuweisen ist. Schlussfolgerungen: Der nach unseren Untersu­chungs­ergebnissen beobachtete fehlende Nachweis eines Einflusses von Glukose- und Insulinkon­zen­trationen auf die Apoptose/Nekroserate der kultivierten Endothelzellen ist also entweder ein methodisches Problem (nicht relevante Endothelzell­sub­popu­lationen, Nachweisverfahren für Apoptose nicht sensitiv genug) oder es besteht in der Tat kein pathophysiologischer Einfluss verschiedener Insulin- und Glukosekonzentrationen auf die Apoptose- und Nekroserate von stimulierten Endothelzellen.

Summary

Background: In patients with severe sepsis/septi­caemia, intensive insulin therapy (IIT) - introduced by van den Berghe et al. in 2001 – represents a new therapeutic principle. Although that study showed a significant reduction in patient mortality through the use of IIT, the findings of further studies (e.g. the VISEP study) were not conclusive. The aim of the present study was to investigate the influence of various concentrations of glucose and insulin on the viability of endothelial cells in vitro. Method: Endothelial cells were isolated from human umbilical veins (HUVEC) and cultured in standard media. Cells from the 3rd - 5th passage were then incubated with insulin (0, 0.005 and 0.01 iE/ml) and/or glucose (0, 5, 10 mmol/ml). The relative percentage of necrotic (propidium iodide) or apoptotic (Annexin) cells were analysed in a flow cytometer. Results: The results obtained revealed no significant influence of various insulin and glucose concentrations on the rate of apoptosis/necrosis of HUVEC cells. Conclusion: The absence of any significant influ­ence of insulin and/or glucose concentrations on necrosis/apoptosis might reflect a methodological problem (i.e. non-relevant endothelial cell subpopulation, insufficient sensitivity of the methodology for the detection of apoptosis) or it might reveal an absence of any influence of insulin and glucose con
