Zusammenfassung
Gerinnungsstörungen gehen von Thrombozyten, plasmatischer Gerinnung, deren Inhibition, Gerinnselbildung oder Fibrinolyse aus. Diagnostik ist hier effektiver und immer preiswerter als ungezielte Therapie. Klinische Situation und Anamnese geben die wesentlichen Hinweise.
Labordiagnostik ergänzt die Einschätzung. Gerinnungstherapie nach Maß soll in angemessener Zeit mit minimalen Nebenwirkungen und geringstem Verbrauch von Ressourcen den notwendigen Erfolg erzielen. Gerinnungstherapeutika stehen für nahezu alle Situationen zur Verfügung. Tagestherapiekosten reichen von mehreren Tausend Euro (Faktor VIIa) bis hinab zu wenigen Cent (orale Antikoagulantien). Rückenmarksnahe Regionalanästhesie wird nur bei intakter Gerinnung empfohlen. Heparine und Acetylsalicylsäure sind hier nur in prophylaktischer Dosis erlaubt.
Summary
Coagulation disorders may originate with platelets, plasmatic coagulation or its inhibition, clot formation or fibrinolysis. Prior diagnostic clarification always leads to more effective results than blind therapy. The clinical situation and patient history provide important information, and laboratory investigations finalise the diagnosis. Optimal coagulation therapy should achieve success with a minimum of side effects and resources and within an acceptable time. Therapeutic agents are available for almost situations. Daily treatment costs range from thousands of euros (factor VIIa) to only a few cents (oral anticoagulants). Central regional anaesthesia should be used only when coagulation is intact, and heparins and aspirin are permitted in prophylactic doses only.
