Zusammenfassung

Hintergrund: Intensivpatienten schei­‑ nen von einer frühenteralen Ernährung zu profitieren. Allerdings ist der Appli­‑ kationsweg Gegenstand anhaltender Diskussionen. Postpylorische Ernährungssonden ermöglichen die enterale Ernährung bei prolongierter Magen-Darmatonie mit erhöhtem gastralem Reflux.


Diese können intraoperativ als Jejunalfisteln oder im Rahmen einer Endoskopie platziert werden. Alternativ besteht die Möglichkeit, minimal-invasiv mit einer elektromagnetischen Methode Corflo®-Tube-Sonden zu legen. Dabei handelt es sich um ein in Deutschland neuartiges System zur bettseitigen Platzierung von postpylorischen Ernäh­rungssonden ohne Endoskopie: die Platzierung erfolgt mit Unterstützung eines Detektors und Monitors, auf dem der Verlauf der magnetisierten Spitze vom Mandrin beim Vorschieben mit dem Ziel der postpylorischen Positionierung abgebildet wird. Material und Methoden: Eine retro­spektive Datenerhebung zur Anlage von Corflo®-Tube-Sonden bei operativen Intensivpatienten sowie Literaturdaten dienen als Grundlage der Diskussion, inwieweit die Technik mit der Corflo®-Tube-Sonde zur postpylorischen Ernäh­‑ rung eine gute Alternative zu aufwändigeren Verfahren, wie zum Beispiel der endoskopische Anlage, darstellt. Ergebnisse: Im Zeitraum von Juli 2008 bis März 2010 wurden 50 Intensivpa­tienten, die frühenteral über eine Ma­‑ gensonde ernährt und trotz Oberkörperhochlage und der Gabe von Prokinetika einen erhöhten Reflux aufwiesen, für die Anlage einer Corflo-Tube®-Sonde ausgewählt. Dabei konnten 45 von 50 Corflo-Tube®-Sonden (90 %) erfolgreich platziert werden. Die Platzierungsdauer betrug im Median 15 Minuten (Spannweite 1-150 Minuten). Literaturdaten zeigen, dass die endoskopische Anlage postpylorischer Sonden eine über 90 %ige Erfolgsrate hat und im Durchschnitt nach ca. 15 Minuten eine erfolgreiche Sondenanlage ermöglicht. Zusammenfassung: Durch ihre einfache Handhabung und schnelle Verfügbarkeit stellen die Corflo®-Tube-Sonden eine Al­‑ ternative zu aufwändigeren Verfahren dar und haben auch als Versuch bei fehlender Möglichkeit der endoskopischen Anlage postpylorischer Sonden, z.B. am Wochenende oder bei logistischen Engpässen, eine gerechtfertigte Indikation

Summary Background: Intensive care patients appear to benefit from early enteral nutrition, although there is continuing discussion on what constitutes the best mode of administration. Post-pyloric nutritional probes permit enteral nutrition in patients with prolonged gastrointestinal atonia and increased gastric reflux. These can be positioned intraoperatively as jejunal fistulae, or during endoscopy. An alternative, minimally invasive pro­‑ cedure, is the electromagnetic supported placement of a Corflo® tube introduced recently into clinical practice in Germany. This system permits bedside placement of nutritional probes without the need for endoscopy, using a detector and a monitor, which maps the path of the magnetic tip of the mandrin as it is ad­‑ vanced towards its post-pyloric position. Materials and methods: A retrospective investigation was performed to collect data on the insertion of Corflo® tube probes in surgical intensive care patients which, together with data from the lit­erature, provided the basis for discussing the suitability of the Corflo® tube probe for post-pyloric nutrition as a good alternative to more demanding procedures, such as endoscopic insertion. Results: Between July 2008 and March 2010, 50 intensive care patients were selected for insertion of a Corflo® tube probe. These patients had been fed through a gastric probe into the upper intestine and, despite elevation of the upper body and administration of prokinetics, had experienced enhanced reflux. 45 of 50 the Corflo® tube probes (90 %) were successfully placed in a median time of 15 min (range 1-150 min). Literature data show that endoscopic insertion of post-pyloric probes is 90 % successful with correct placement achieved in a mean time of ca. 15 min. Conclusion: Easy to handle and rapidly available, the Corflo® tube probe offers an alternative to more time-consuming procedures. It is also justified to attempt this procedure whenever endoscopic insertion of a post-pyloric probe is impracticable, for example, during the weekend or in the face of logistical bottlenecks.
