Zusammenfassung
Ein Neuromonitoring in der Anästhesie kann zur Erfassung von medikamenteninduzierten Effekten auf das Zentralnervensystem (Hypnosetiefenbestimmung, Barbituratkoma) oder zur Überwachung der funktionellen Organintegrität (Ischä‑
miedetektion in der Karotis- und Skoliosechirurgie, Identifikation von Nerven und Überwachung ihrer Funktion) ge‑
nutzt werden.
Hypnosetiefenmonitore beruhen überwiegend auf einer prozessierten Auswertung des Elektroenzephalogramms mithilfe komplexer Algorithmen. Zur Überwachung von Nerven und Nervenbahnen werden überwiegend evozierte Potentiale eingesetzt. Eine Zulassung zur Überwachung der Anästhesiewirkung besitzen der Bispectral-Index und der Narcotrend-Index. Primäres Ziel des Hypnosetiefenmonitorings ist nicht die Einsparung von Anästhetika, sondern das Ansteuern individuell und situationsgerecht optimaler Anästhesielevel. Die Anwendung von Hypnosetiefenmonitoren kann zu einer Reduktion von Wachheit (Awareness) und postoperativer Übelkeit und Erbrechen sowie zu einer erhöhten Kreislaufstabilität beitragen. Desobliterationen an der A. carotis interna unter Allgemeinanästhesie sollen regelhaft mit einem Neuromonitoring zur Detektion klemmbedingter Ischämien erfolgen. Bei geeigneter Schulung können Anästhesisten durch den Einsatz von Neuromonitoring eine Erweiterung des Leistungsspektrums und eine Erhöhung der Patientensicherheit erzielen.
Summary
Intraoperative neurophysiological monitoring can be used to determine the effects of drugs on the central nervous system (depth of hypnosis, barbiturate-induced coma) or to monitor the functional integrity of the organs (detection of ischaemia in carotid or scoliosis surgery, or to identify and monitor cerebral or peripheral nerves). Depth-of-hypnosis monitors employ complex algorithms to analyse the patient´s electroencephalogram. The Bispectral Index and the Narcotrend-Index are an approved means
for „assessing the hypnotic effects of general anaesthetics and sedatives”. Primarily depth of hypnosis monitoring aims to achieve an optimal level of anaesthesia for the individual situation / person but not to reduce the amount of drug used. Thus, using depth-of-hypnosis monitors may help to achieve a reduction in awareness, postoperative nausea and vomiting as well as stabilization of the circulation.
Carotid endarterectomy under general anaesthesia necessitates regular use of
intraoperative neurophysiological moni‑
toring to detect clamping-induced cerebral ischaemia. This article describes several procedures. For the monitoring of nerves and nerve pathways evoked potentials are the method of choice. After appropriate training, anaesthesiologists may extend their service spectrum and improve patient safety through the use of intraoperative neurophysiological monitoring.
