Zusammenfassung

Hintergrund: Eine relativ häufige Ne­benwirkung der elektrokonvulsiven The­- rapie (EKT) ist eine postiktale Agitation (PIA) mit einer Häufigkeit von ca. 12% während einer modifizierten (d.h. unter Kurznarkose und Muskelrelaxation durchgeführten) EKT-Behandlung.


Aus­gehend von der Beobachtung, dass PIA bei nicht modifizierter EKT, d.h. ohne anästhesiologische Maßnahmen, wesentlich häufiger vorkommt als bei modifizierter EKT, erscheint eine ausreichende Narkosetiefe von Vorteil zu sein. Der bispektrale Index Score (BIS) wird durch eine spezifische elektroenzephalographische Frequenzanalyse ermittelt und eignet sich zur Quantifizierung des Narkosentiefenverlaufs bei der EKT. Methoden: Ziel dieser Studie war es zu untersuchen, ob die Erfassung der anfänglichen, maximalen Tiefe einer Thiopentalnarkose während der EKT-Behandlung als Instrument zur Vorhersage des Auftretens einer PIA verwendet werden kann. Wir werteten retrospektiv die klinischen Aufzeichnungen von 53 Patienten aus, die insgesamt 480 einzelne EKTs zwischen 2011 und 2014 am Zentralinstitut für Seelische Gesundheit in Mannheim erhielten. Um unsere Hypothese zu untersuchen, verwendeten wir eine multivariate, wiederholte logistische Regressionsanalyse mit dem Auftreten einer PIA als abhängige Va­riable und dem tiefsten BIS-Wert vor der Anfallsinduktion (BIS-Nadir), BIS bei Anfallsinduktion, Geschlecht, Alter, Diagnose und Stimulationsdosis, Dosis von Succinylcholin, Elektrodenplatzierung und Anfallsqualitätsparametern als unabhängige Variablen. Ergebnis: In Übereinstimmung mit un­serer Hypothese konnten wir zeigen, dass der BIS-Nadir signifikant das Nicht-Auftreten einer PIA vorhersagt. Schlussfolgerung: Eine BIS-überwachte Narkoseführung könnte eine vielver­sprechende Strategie für die PIA-Prävention bei der EKT sein.

Summary Background: A relatively common side effect of electroconvulsive therapy (ECT) is the postictal agitation syndrome (PIA), with an incidence of about 12% during a modified ECT-course. Based on the observation that PIA is considerably more frequent in unmodified ECT with­out anaesthesia than in modified ECT conducted under general anaesthesia, ensuring an adequate depth of anaesthesia appears to be beneficial. The bispectral index score (BIS) is a specific electroencephalographic frequency ana­lysis and very suitable to quantify the depth of anaesthesia. Methods: Our aim was to examine if assessing the initial depth of thiopental anaesthesia by applying BIS-monitoring during an ECT session can be used as a tool to predict PIA. To test the hypothesis whether a low BIS nadir predicts the non-occurrence of PIA, we used a multivariate repeated-measurement logistic regression analysis with occurrence of PIA as the dependent variable, whereas BIS nadir, BIS at seizure induction, sex, age, diagnosis, stimulation dose, dose of succinylcholine, electrode placement and seizure quality parameters were independent variables. We retrospectively evaluated the clinical records of 53 patients who received a total of 480 ECTs between 2011 and 2014 at the Central Institute of Mental Health in Mannheim, Germany. Results: Consistent with our hypothesis, the lowest BIS value after induction of anaesthesia significantly predicted the non-occurrence of PIA. Conclusion: We conclude that monitor­ing BIS values allowing the adjustment of anaesthesia accordingly might be a promising strategy to prevent PIA.
