Zusammenfassung

Lokalanästhetika sind aus der klinischen Praxis nicht mehr wegzudenken. Unabhängig von ihren physikochemischen Eigenschaften und ihrer chemischen Struktur blockieren sämtliche Lokalanästhetika spannungsgesteuerte Natrium­kanäle (Na+-Kanäle) und verhindern so die Erregungsleitung in peripheren Nerven.


Darüber hinaus sind die Substanzen durch eine Vielzahl weiterer Wirkungen auf (sub-)zellulärer Ebene charakterisiert. Trotz der Entwicklung von Lokalanästhetika mit deutlich ge­ringerem toxischem Potenzial ist die systemische Intoxikation nach wie vor lebensbedrohlich. Diese entsteht durch eine unselektive Blockade neuronaler und kardialer Na+-Kanäle bei exzessiver systemischer Akkumulation und äußert sich klinisch in einer Beeinträchtigung des zentralen Nervensystems sowie des kardiovaskulären Systems. Die klinische Bedeutung lokaler gewebetoxischer Erscheinungen wird kontrovers diskutiert, zumal diese in vielen Fällen inapparent verlaufen.

Summary Local anaesthetics are widely used in contemporary clinical practice. Regardless of their specific physicochemical properties and chemical structures, all local anaesthetic agents block neuronal voltage-gated sodium channels, suppressing conduction in peripheral nerves. Furthermore, these agents are characterised by numerous (sub-)cellular effects. Despite the fact that local anaesthetics with markedly decreased toxic potential have been developed, systemic intoxication still may be life-threatening. Amongst other things, this severe complication is the result of an unselective block of neuronal and cardiac sodium channels following excessive systemic accumulation, impairing central nervous and cardiac function. In contrast, the clinical impact of local anaesthetic tissue toxicity is controversial, as in many cases there is a lack of clinical symptoms.
