Despite the availability of multimodal therapy options ARDS (Acute Respiratory Distress Syndrome) is still a complex intensive medical disease associated with a constant high mortality rate.
Whereas infectious causes most often predominate, less often occurring differential diagnoses are taken into consideration to a lesser degree despite absent proof of infection. We now report the case of a 57-year-old female patient with serious ARDS and a treatment indication which included VV-ECMO therapy. The diagnostic confirmation of the underlying cause consisting of a bilateral pulmonary mucinous adenocarcinoma of the lung was discovered to be the rare cause of ARDS only in the later course, after an open lung biopsy with subsequent histopathological examination.
