Zusammenfassung: Die perioperative Optimierung des Volumenstatus des Patienten ist integraler Bestandteil einer guten Narkoseführung. Seit mehr als vierzig Jahren wird die Diskussion über eine optimale Flüssigkeitstherapie emotional und kontrovers geführt und wirft nach wie vor eine Reihe ungelöster Fragen auf.
Die Übersichtsarbeit beschreibt die historische Entwicklung der vorherrschenden Prinzipien der Flüssigkeitstherapie, beschäftigt sich mit den (patho-)physiologischen Grundlagen, der Kristalloid-Kolloid-Kontroverse sowie den Unterschieden und Auswirkungen balancierter versus nicht-balancierter Elektrolytlösungen. Abschließend werden tradierte und neuere („goal-directed“) Konzepte zur optimierten Flüssigkeitstherapie beleuchtet.

Summary: Perioperative optimization of the patients fluid status is an integral part of good anaesthesia practice. For more than forty years, there has been an emotional and controversial discussion about what constitutes optimal fluid therapy, and many questions have yet to be answered. This review first describes the historical development of fluid management principles and then takes a look at the (patho-) physiological background. New light is shed on the “crystalloid- colloid debate”, and the impact of balanced versus nonbalanced fluids is illustrated. Finally, traditional and more recent “goal-directed” fluid replacement strategies are reviewed.
