Zusammenfassung: Die Basistherapie von Patienten mit neuronaler Schädigung zielt darauf ab, den intrakraniellen Druck (ICP) unterhalb von 20?mmHg und den zerebralen Perfusionsdruck innerhalb des Bereichs von 50-70 mmHg zu halten. Der erhöhte ICP kann über eine Gabe von Osmodiuretika (Mannitol, hypertone NaCl-Lösung), eine Vermeidung hypoosmolarer Lösungen und eine Optimierung der Lagerung therapiert werden.
Falls diese Maßnahmen nicht ausreichend sind, besteht die Option eines Barbituratkomas (bei stabiler Hämodynamik), einer milden Hypothermiebehandlung, einer milden Hyperventilation und der Dekompressionskraniektomie.

Summary: In patients with neuronal damage the main target of therapy is to keep the intracranial pressure below 20mmHg and the cerebral perfusion pressure between 50-70mmHg. An increased intracranial pressure can be treated with hypertonic solutions or mannitol, whereas hypotonic solutions should be avoided. Additionally, a deeper sedation with barbiturates, mild hypothermia, and mild hyperventilation can be applied as therapy. The value of a decompressive craniectomy in patients with persistent high intracranial pressure is still under discussion.
