Zusammenfassung: Die anästhesiologische Versorgung von Kindern mit Lippen-Kiefer-Gaumen-Spalten gilt in Europa als sicher. In Entwicklungsländern, wie z.B. Nepal, fehlen jedoch wesentliche Voraussetzungen zur exakten präoperativen Risikoevaluierung und Patientenaufklärung.
Die medizinisch-technischen Voraussetzungen für eine sichere Anästhesie und eine adäquate postoperative Überwachung sind sehr eingeschränkt und in Ab-hängigkeit vom jeweiligen Hospital heterogen. Wir berichten über unsere Erfahrungen aus zwei Hospitälern, stellen unsere Behandlungsstrategie vor und verweisen auf die Notwendigkeit einer engen Teamarbeit für diese anspruchsvollen Operationen unter eingeschränkten Bedingungen.

Summary:Anaesthesia for surgical correction of a cleft lip and palate is a safe procedure in Europe. In devel-oping countries,such as Nepal,the means and possibi-lities for accurate preoperative risk evaluation and adequate patient education are often limited. Furthermore, medical and technical conditions vary considerably between the individual healthcare institutions and many hospitals lack the medical equip-ment that is needed to perform safe anaesthesia and to monitor the patients postoperatively.We report on our experiences in two Nepalese hospital settings and pre-sent our therapeutic concept for the provision of safe anaesthesia in cleft lip and palate surgery under restricted conditions.Special emphasis is placed on the importance of close teamwork as well as on the neces-sity to develop an algorithm for dealing with unexpect-ed technical challenges.
