Zusammenfassung: Die zunehmende Verknappung der Ressourcen und die Einführung des DRG-Systems in die Akutkliniken erfordert eine Prozessoptimierung durch alle Beteiligten, um die notwendigen Leistungen für den Patienten ökonomisch zu erbringen. Die Etablierung von Clinical Pathways, in denen der Behandlungsablauf des Patienten von der Aufnahme bis zur Entlassung definiert ist, trägt wesentlich zu einer Prozessoptimierung bei.


In zahlreichen Untersuchungen konnte gezeigt werden, dass die konsequente Anwendung von Clinical Pathways zu einer deutlichen Liegezeitverkürzung und damit zu ökonomischen Vorteilen bei gleicher Qualität führt. Clinical Pathways können nur von den prozessverantwortlichen Abteilungen, die den Patienten von der Aufnahme bis zur Entlassung betreuen, definiert werden. Die Entwicklung von Clinical Pathways muss immer interdisziplinär und interprofessionell erfolgen. Die klinischen Abläufe, die nicht durch die Prozessverantwortlichen erbracht werden, lassen sich durch Standard Operating Procedures (SOPs) definieren und als Modul in den Clinical Pathway einfügen. Die Anästhesie ist nahezu bei allen operativen Patienten an der Versorgung beteiligt. Da die Anästhesiologie und Intensivmedizin wichtige Schnittstellenfächer darstellen und an den verschiedensten Abschnitten des Behandlungsweges eines Patienten mitwirken, müssen sie in Zukunft vermehrt Module in Form von Standard Operating Procedures als Bestandteile von Clinical Pathways anbieten können. In der vorliegenden Publikation wird das formale Vorgehen zur Entwicklung von Standard Operating Procedures dargestellt.

Summary Summary: Increasing shortage of resources in general hospitals and the introduction of the DiagnosisRelated Group system call for an enhanced economic performance of clinical procedures that is to be achieved through an optimisation of processes by all those who are involved in the provision of health care services. The implementation of clinical pathways, which define the entire process of health care provision from a patient’s hospital admission to discharge, contribute substantially to the optimisation of processes. Numerous studies have shown that the consistent use of clinical pathways is economically advantageous as it leads to a shorter length of stay in hospital, while the quality of treatment remains the same. Clinical pathways may only be defined by those departments that are responsible for organizing and delivering health care services to the patient from the point of hospital admission to discharge. The development of clinical pathways must always be achieved by an interdisciplinary approach and close cooperation of the different specialties and professions. Clinical procedures that are not performed by those who are responsible for process management and optimisation can be defined as Standard Operating Procedures (SOPs) and included in a clinical pathway as a module. The Department of Anaesthesiology is usually taking part in the clinical care of almost any surgical patient. Since anaesthesiology and critical care medicine are important "interface disciplines" and are involved at different stages in a patient’s course of treatment, they are expected to define an increasing number of standard operating procedures to be used as modules in the clinical pathways. This publication describes how standard operating procedures are developed.
