Zusammenfassung
Die akute systemische Lokalanästhetika-Intoxikation ist eine seltene, aber unter Umständen lebensbedrohliche Komplikation der Regionalanästhesie. Diese Komplikation zu vermeiden, die Frühwarnzeichen und die Symptome zu erkennen und die lebensrettenden Erstmaßnahmen einzuleiten, ist für den Anwender von potentiell toxischen Dosierungen von Lokalanästhetika essentiell.
Einen besonderen Schwerpunkt dieses Beitrags bilden die Empfehlungen des „Wissenschaftlichen Arbeitskreises Regionalanästhesie“ der Deutschen Gesellschaft für Anästhesiologie und Intensivmedizin (DGAI) zur adjuvanten Lipid-Behandlung aus dem Jahre 2009 ebenso wie die aktuellen Handlungsempfehlungen der American Society of Regional Anesthesia and Pain Medicine (ASRA) zur Prävention, Diagnose, Therapie und adjuvanten Pharmakotherapie der Lokalanästhetika-Intoxikation.
Summary
Acute local anaesthetic systemic toxicity is a rare but potentially fatal complication of regional anaesthesia. For those who administer local anaesthetics at po‑
tentially toxic doses it is essential that they have the expertise to recognize warning signs and initial symptoms and to initiate life-saving countermeasures. A major emphasis of this review are the 2009 recommendations of the regional anaesthesia research panel of the German Society of Anaesthesiology and Intensive Care Medicine (DGAI) regarding lipid therapy, and the current guidelines of the American Society of Regional Anesthesia and Pain Medicine (ASRA) on the prevention, diagnosis, treatment and adjuvant pharmacotherapy of local anaesthetic systemic toxicity.
