Zusammenfassung
Ein Notarzteinsatz bei einem Palliativpatienten ist eine besondere Herausforderung, wobei es besonders auf die Einschätzung der Prognose und des mutmaßlichen Patientenwillens ankommt. Ursache der Alarmierung sind meist akute Symptom-Exazerbationen (Schmerzkrise, akute Luftnot, Blutung, drohender oder eingetretener Kreislaufstillstand) oder wenn eine akut auftretende Situation die Angehörigen überfordert.
Eine Symptom-Exazerbation muss individuell unter Beachtung der aktuellen Situation und des Patientenwillens bewertet und angemessen versorgt werden. Darüber hinaus ist es Aufgabe des Notarztes, den Angehörigen die Nachricht vom unmittelbar bevorstehenden oder eingetretenen Tod des Patienten einfühlsam zu überbringen. Eine Patientenverfügung ist häufig nicht ausreichend individualisiert und damit nicht immer hilfreich. Hier kann eine vorausschauende Betreuungsplanung (Advance Care Planning) den Rettungsdienst in der akuten Situation unterstützen.
Summary
A palliative patient is a special challenge for the emergency physician with prior respect to the assessment of prognosis and the presumed will of the patient. Most acute symptom exacerbations (pain crisis, acute shortness of breath, bleeding, imminent or occurring circulatory arrest) as well as mental or physical overload of relatives are cause of alarm. A symptom exacerbation must be assessed individually in accordance with the present situation and the will of the patient, and adequately managed. In case of imminent or manifest death, the emergency physician must inform the relatives in an empathetic manner. An advance directive is often not sufficiently individualised and thus not always helpful. Advance care planning can support the rescue service in the acute situation.
