Zusammenfassung

Zusammenfassung: Mit den Globaltesten der plasma-tischen Gerinnung ist nur eine eingeschränkte Aussage über die Funktionsfähigkeit der komplexen Interaktion zellulärer und plasmatischer Bestandteile der Hämostase möglich. Wichtig ist die Ergänzung durch klinische und anamnestische Informationen.Deutliche Fortschritte wurden in den letzten Jahren hinsichtlich Präzision, Sensitivität und auch Vergleichbarkeit der unterschiedlichen Reagenzien erzielt.


Teste zur Erfassung der primären Hämostase müssen noch in prospektiven Studien in ihrer klinischen Wertigkeit gesichert werden.Vollblutmethoden bieten sich wegen schneller Ergebnisse und Erfassung der Interaktionen in Notfallsituationen auch zur Steuerung einer spezifischen Hämotherapie mit plasmati-schen oder zellulären Blutbestandteilen an.

Summary Summary: Plasmatic screening tests are of limited value to discover suspected dysfunction in the highly complex interaction between cells and plasma factors involved in hemostasis. It is of great importance to supplement laboratory results with acute clinical fin-dings and information from the patient‘s medical history. For assessment of primary hemostasis in vivo bleeding time has to be performed as standardized as possible. Whole-blood methods might be helpful in emergency situations due to the rapid and simple measurement procedure. These methods allow in particular the monitoring of specific hemotherapy with plasma fac-tors or cellular blood components. For the future the concept of a global test ist unreali-stic because of the complex demands such a test would have to fulfil for screening, specific assessment, moni-toring of therapy, and hemostatic management in intensive care units.The use of specified methods opti-mized for singular specific demands might enable a more subtle assessment of the individual patient. Furthermore practical approaches of monitoring should be evaluated for the many currently available options of drug interventions having an effect on the hemostatic system, particularly in view of the need of monitoring patients being at risk of bleeding. In con-clusion for prediction of bleeding or thrombosis risks the family history, the patients history, age, current complaints and problems,as well as cofactors possibly enhancing morbidity will need to be inquired and thoroughly analyzed in context with laboratory parameters.
