Zusammenfassung: Die Spinalanästhesie hat in den letzten Jahren trotz bekannter, teils gravierender Komplikationen zunehmendes Interesse gefunden.Im vergangenen Jahrzehnt ist das TNS-Syndrom (=Transitorische neurologische Symptome), das einer radi-kulären Reizung der lumbalen Nervenwurzeln entspricht, zunehmend in die Diskussion gekommen.


Die Spinalanästhesie wirkt über eine Blockade der spinalen Nervenwurzeln auf ihrem Verlauf im Liquorraum. Je nach Ausdehnung ist sie von einer Sympathikolyse begleitet, die hämodynamische Konsequenzen hat. Nach der Höhe des Blocks unterscheidet man zwischen Sattelblock,mittelhoher (bis ca.T 10) und hoher Spinalanästhesie (bis T 4). Höherreichende Spinalan-ästhesien strebt man heute nicht mehr an. An technischen Varianten sind noch Katheter-Spinalanästhesien und die CSE-Methode (Combined Spinal Epidural) in Gebrauch.Weiterhin kennt man die einseitige bzw.sei-tenbetonte Spinalanästhesie sowie die fraktionierte Technik. Die Auswahl der Lokalanästhetika zur Spinalanästhesie muß neben der angestrebten Wirkungsdauer auch das Potential der Mittel zur Auslösung des TNS-Syndroms berücksichtigen. Als Zusatz zum Lokalanästhetikum besitzt nur das Morphin eine Zulassung zur intrathekalen Anwendung. Als Punktionskanülen sollten heute zur Verhü-tung des Liquordrainage-Syndroms (postspinaler Kopfschmerz,Hirnnervenparesen,Hörstörungen,sehr selten intrakranielle Blutungen) möglichst dünne Nadeln verwendet werden. Pencilpoint-Kanülen tragen ebenfalls zur Vermeidung dieses Syndroms bei, sind aber in der Handhabung anspruchsvoller. Als Zugänge zum Spinalkanal konkurrieren der mediane, der paramediane und der nach Taylor (eine Sonderform des paramedianen). Der Mediane eignet sich hauptsächlich für sehr adipöse Patienten. Die Punktionstechnik soll die taktile Identifizierung der perforierten Strukturen zulassen. Als Komplikationen der Methode werden neben dem Liquordrainage-Syndrom Herz-Kreislauf-Komplikationen, Miktionsstö-rungen, das TNS-Syndrom (s. o.) und Raumforderungen im Wirbelkanal, als vermeintliche Komplikation die Impotenz besprochen. Als Indikationen für die Methode bieten sich neben der Sectio caesarea,die eine hohe Spinalanästhesie erfordert, alle Eingriffe unter dem segmentalen Niveau von T 10 an.

Summary Summary:Though spinal anaesthesia may give rise to well-known and sometimes severe complications, nowadays it is used increasingly.In the last decade,the TNS-syndrome (TNS=transitory neurological symptoms) has been described that resembles a radicular irritation of the lumbar nerve roots.Spinal anaesthesia is caused by a block of the nerve roots during their course through the CSF. Depending from its extent, it leads to a sympathetic block which has consequences for haemodynamics. According to its spread, one can distinguish between saddle block, medium high (to T 10) and high (to T4) spinal anaesthesia. A higher extent than T 4 is no more intended nowadays.Spinal catheter techniques and the Combined Spinal Epi-dural (CSE)-method are usual. Furthermore, lateral spread („one-side-block”) can be achieved, and an injection of incremental doses using bupivacaine results in a long-duration anaesthesia of limited spread („fractionated technique”). The choice of the local anaesthetic depends on the time necessary for the operation and should notice the risk of TNS. The sole opioid which has the German approval for subarachnoid injection is morphine. For prevention of CSF leakage syndrome (PSH, cerebral nerve palsy,auditive losses and very rarely intracranial bleeding) the needles used should be very thin.Pencil-point-needles contribute to minimizing this risk, but may be difficult to handle.As approaches to the spinal canal, the median, the paramedian and the Taylor-approach (a variant of the paramedian approach) compete. The median approach has advantages in obese patients.The technique should allow for precise feeling of the structures which just are passed by the needle. As untoward side effects of the method, besi-des the CSF-leakage there are discussed haemodyna-mic disturbances, voiding problems, the TNS-syndrome,and space occupying lesions of the spinal canal.As indications for the method besides caesarean section, which needs a high spread,are considered all procedu-res under the level of T 10.
