Zusammenfassung: Mit einer pharmako-ökonomischen Modellrechnung werden die zu erwartenden Aufwachzeiten und entstehenden Medikamentenkosten abhängig von der Auswahl der Anästhetika und dem Verhältnis ihrer Kombination untersucht. Anhand bekannter pharmakokinetischer und –dynamischer Eigenschaften und ihrer Interaktion wurden die für suffiziente Narkosetiefe erforderlichen Dosierungen von volatilen Anästhetika und Opiaten sowie der Konzentrationsabfall bis zum Aufwachen berechnet.
Die Erhaltungskonzentrationen unterschiedlicher Anästhetika-Kombinationen konnten hinsichtlich Aufwachzeiten und Kosten optimiert werden. Bei 120 Minuten Anästhesiedauer ergeben sich die kürzesten Aufwachzeiten (2,8 Min) bei Verwendung von Desfluran und Remifentanil; jedoch entstehen im Vergleich zu Isofluran und Fentanyl (6,5 Min Aufwachzeit) um DM 30,- ( ¡¡ 15,-) höhere Medikamentenkosten (Stand: November 2001).

Summary: Using a pharmaco-economic analysis we investigate awakening times and drug costs depending on selection of the volatile anaesthetic and opioid and their combination. Based on their known pharmaco-kinetic and -dynamic properties and their interaction, the dosage of volatile anaesthetics and opioids required for sufficient depth of anaesthesia and decrease in concentrations until awakening were calculated. Introduction of minimum awakening times and minimum drug costs as secondary conditions in addition to sufficient depth of anaesthesia allowed optimisation of the maintenance concentrations. Minimum awakening times (2.8 min for 120 min anaesthesia duration) were calculated for desflurane / remifentanil; but compared with isoflurane / fentanyl (6.5 min awakening time) drug costs were elevated by DM 30,- ( ¡¡15,-) (costs November 2001).
