Indications for mechanical ventilation reconsidered

Zusammenfassung

Zusammenfassung: Die Beatmung kann mittels endotra-chealer Tuben invasiv oder nichtinvasiv mit Atemmasken oder Atemhelmen erfolgen. Bei der Auswahl des Beatmungsverfahrens müssen die jeweiligen Eigenschaften z.B.bezüglich der Sicherheit des Atemweges,der Risiken von Baro- und Volutrauma und ventilatorassoziierter Pneumonie berücksichtigt und die Kontraindikationen der nichtinvasiven Beatmung bedacht werden.


Die Einschlusskriterien und damit die Indikationen zur Beatmung sind in verschiedenen Studien unterschiedlich festgelegt.Die Entscheidung für eine Beatmungstherapie wird anhand des klinischen Bildes,der Grunderkrankung und der Parameter von Hypoxie (PaO2, SaO2), Hyperkapnie (PaCO2) sowie dem Ausmaß einer Azidose (pHa) getroffen. Die Indikation für eine nichtinvasive Beatmung ist zur Behandlung der akuten respiratorischen Insuffizienz bei akut exazerbierter chronisch-obstruktiver Lungenerkrankung (COPD) gesichert. Sie ist ebenfalls zur Therapie des kardialen Lungenödems indiziert. Dabei muss die druckunterstützte, nichtinvasive Beatmung bis zum Ausschluss kardialer Ischämien vorsichtig angewendet werden, um das Herzinfarktrisiko zu minimieren. Der Nutzen der nichtinvasiven Beatmung konnte weiterhin für organtransplantierte oder aus anderer Ursache im-munsupprimierte Patienten mit ALI (acute lung injury) / ARDS (adult respiratory distress syndrome) gezeigt werden. Ermutigende Ergebnisse der nichtinvasiven Beatmungstherapie liegen ebenfalls bei akuter respiratorischer Insuffizienz anderer Ätiologien vor. Bei chronischer respiratorischer Insuffizienz ist die nicht-invasive Beatmung neben der Behandlung der Dyspnoe besonders zur Therapie von Schlafstörungen, Müdigkeit, Konzentrationsstörungen oder Kopfschmerzen hilfreich.

Summary Summary: Mechanical ventilatory support can be a-chieved invasively using endotracheal tubes or non-inva-sively using face masks and respirator helmets.The techniques differ from each other,e.g.in terms of the safety of the airway or the risks of baro- and volutrauma and ventilator-induced pneumonia, and should therefore be chosen carefully according to their individual properties.For a safe application of non-invasive ventilation,contraindi-cations also have to be taken into account. In studies of respiratory failure, different inclusion crite-ria and indications for mechanical ventilatory support are used.The decision is made considering the clinical condi-tion,the underlying disease,and the parameters of hypo- xia (PaO2,SaO2),hypercapnia (PaCO2) and acidosis (pHa). Non-invasive ventilation is indicated in the therapy of acute respiratory failure in acute exacerbation of chronic obstructive pulmonary disease (COPD). It is also indica-ted in the therapy of cardiogenic pulmonary oedema.To minimize the risk of myocardial infarction, care must be taken when applying non-invasive pressure support ventilation in these cases before myocardial ischaemia is ruled out.The benefit of non-invasive ventilation was shown in patients undergoing solid organ transplantation and in other immunosuppressed patients with acute lung injury / adult respiratory distress syndrome (ALI/ARDS).There are also encouraging results with non-invasive ventilatory therapy of acute respiratory failure of some other aetio-logy. In chronic respiratory failure, non-invasive ventilation is important for the treatment of dyspnoea, but also to reduce nocturnal sleep disruption, cognitive dysfunction, excessive fatigue and morning headache.
