Zusammenfassung
Zusammenfassung: Die Einführung des pauschalisierten Vergütungssystems der Diagnosis-Related Groups (DRG) führte in den Krankenhäusern zu drastischen Verweildauerverkürzungen. Die Umsetzung der als notwendig erkannten Verweildauerverkürzung dauert bis heute an und betrifft auch den Funktionsbereich Operationssaal.
Hier fallen durch die hohe Personaldichte 30-40% der gesamten Fallkosten einer operativen DRG an. Zur optimalen Nutzung der Ressourcen ist es sinnvoll, klinische Behandlungspfade und standardisierte Behandlungen zu definieren. Vor der Einführung müssen die Standards ökonomisch validiert und geprüft werden. Hier bietet das Programm „SOPtool.xls“ in Kombination mit dem DRG-Kalkulationstool eine Möglichkeit, Simulationen durchzuführen. So ist es dem Anwender möglich, auf einfache Weise festzustellen, ob das geplante oder etablierte Verfahren in Vergleich zur InEK-Kostenmatrix einen positiven Erlös erzielt. Neben dieser Betrachtung können die mit dem Tool berechneten Ergebnisse auch als Argumentationshilfe bei innerbetrieblichen Verhandlungen über Prozesszeiten verwendet werden.

Summary: The introduction of the lump-sum remuneration system of the Diagnosis-Related Groups (DRG) led to drastic reductions of times spent in hospitals. The implementation of the
reduction of the times spent, which was recognised as being necessary, is still continuing and also affects the operating theatre functional area, where 30-40% of the total case costs of an operative DRG are caused as a result of the high personnel density. For optimum use of the resources, it is sensible to define clinical treatment paths and standardised
treatments. Before the introduction, the standards have to be economically validated and audited. This is where the "SOPtool.xls" programme in combination with the DRG calculation tool provides a possibility of carrying out simulations. For example, it is possible for the user to establish easily whether the planned or established procedures achieve a positive income compared with the InEK cost matrix. Alongside this observation, the results calculated with this tool can also be used as an aid in argumentation for in-house negotiations about process times.
