Zusammenfassung
Die Kombination von Opiaten mit weiteren Anästhetika wie z.B. i.v. Hypnotika oder Benzodiazepinen ist eine weit geübte Praxis, deren pharmakologische Interaktion daher von besonderem Interesse ist. Diese wird in jüngster Zeit mit neuen Monitorverfahren direkt am Arbeitsplatz visualisiert.
In der Literatur werden überwiegend zwei Analyseverfahren verwandt: die Isobolen- und die Response Surface Methode. Ziel der Arbeit ist es, diese Methoden, ihre Voraussetzungen und Unterschiede darzulegen. Für die Isobolenmethode werden für die Interaktion von Opiaten mit Propofol bzw. Benzodiazepinen sowohl additive als auch infraadditive und supraadditive (=synergistische) Interaktionen berichtet. Für die Response Surface Methode werden nur synergistische Interaktionen gefunden. Die Gründe hierfür sollten untersucht werden. Eine Ursache könnte die Auswahl des Modellansatzes der Response Surface sein, die bei allen untersuchten Arbeiten eine allgemeine additive Response Surface nicht berücksichtigt.
Summary
The combination of opioids with other anaesthetics like i.v. hypnotics or benzodiazepines is common use in clinical practice giving special interest in studying their pharmacological interaction. Very recently a new monitoring has been offered visualising such interaction immediately at the anaesthetic work place. Anaesthetic literature reports mainly two methods for analyzing drug-drug interaction, the isobole method and the response surface method. The aim of this work is to characterize these methods, their preconditions and their differences. For the isobole method all types of interactions – additive, infraadditive or supraadditive (synergistic) – are reported. For the response surface method only synergistic interactions are reported. There is need to investigate this finding. One reason that only synergistic response surfaces are found could be that the functions used for modelling the response surface do not include a general additive response surface.
