Nosokomiale Infektionen in der Intensivmedizin

Zusammenfassung

Nosokomiale Infektionen in der Intensivmedizin beeinflussen nachhaltig die Morbidität und Mortalität. Da in der Intensivmedizin häufig invasive diagnostische und therapeutische Verfahren angewendet werden, sind nosokomiale Infektionen oft mit diesen vergesellschaftet.

Die häufigsten nosokomialen Infektionen auf Intensivstationen sind die beatmungsassoziierte Pneumonie, die Zentralvenenkatheter-bedingte Sepsis, die Harnwegsinfektion durch Harnwegskatheter sowie die postoperative Wundinfektion. Maßnahmen, welche das intestinale Mikrobiom des Patienten (z. B. Stressulkusprophylaxe) beeinflussen, haben direkte Auswirkungen auf das Auftreten von nosokomialen Infektionen. Neben spezifischen Präventionsmaßnahmen zur Verhinderung von einzelnen nosokomialen Infektionen bleibt die strikte Händedesinfektion vor und nach jedem Patientenkontakt eine allgemeingültige Präventionsmaßnahme für alle nosokomialen Infektionen.

Summary

Nosocomial infections in critically ill patients have a severe impact on individual morbidity and mortality. Invasive diagnostic and therapeutic measurements are often performed in critically ill patients and, therefore, nosocomial infections are of frequent occurrence in intensive care units (ICU). Ventilator-associated pneumonia, catheter-line-associated blood-stream infection, urinary tract infection, and surgical site infection are the most common noso-comial ICU infections. Procedures with influence on the microbiome of the intestinal tract (e. g., stress ulcer prophylaxes) have a direct impact on the incidence of nosocomial infections. Not only specific methods designed to prevent individual nosocomial infections but absolute compliance with the hygiene rules including hand disinfection before and after every patient contact are effective preventive measures for all nosocomial infections.
