Zusammenfassung

Remimazolam ist ein Benzodiazepin und besitzt innerhalb dieser Gruppe eine einmalige Kinetik: es wird organ­unabhängig innerhalb von Minuten in einen nichtaktiven Metaboliten verstoffwechselt. Die ultrakurze Wirkdauer erklärt sich durch den Abbau über ubiquitär vorkommende unspezifische Gewebsesterasen.

Dieser Metabolismus verleiht auch dem ultrakurzwirksamen Opioid Remifentanil und dem Betablocker Esmolol ihre spezifischen, in der klinischen Anästhesiologie geschätzten Eigenschaften. Damit ist es erstmals möglich, die spezifischen Vorteile von Benzodiazepinen in der Narkoseführung auch bei kurzen und sehr kurzen Eingriffen zu nutzen. Besonders kardial schwer vorerkrankte Patienten können von den im Vergleich mit Propofol und Barbituraten geringeren kardiovaskulären Nebenwirkungen profitieren. Ziel des vorliegenden Artikels ist es, das momentan verfügbare Wissen zu diesem, in der Europäischen Union aktuell noch in der klinischen Prüfung befindlichen Medikament zusammenzustellen und mögliche Anwendungsgebiete zu erörtern.

Summary

Remimazolam is a new short-acting benzodiazepine; within the group of benzodiazepines, it possesses unique kinetics. It is rapidly converted by esterases into a non-active metabolite.

The same mechanism terminates the action of the ultrashort-acting opioid remifentanil and the beta blocker esmolol, enabling their specific and well appreciated characteristics in clinical anaesthesiology. For the first time, it is now possible to employ the advantages of benzodiazepines for short and very short surgical and diagnostic procedures. Especially patients with severe cardiac impairment may benefit from remimazolam’s reduced cardiovascular side effects compared to alternative hypnotic agents like pro-
pofol. Remimazolam could therefore be advantageous in the treatment of patients with cardiac impairment. The present article aims to review the current knowledge about this drug, which is still in clinical investigation in the European Union, and discuss potential fields of application.
