Zusammenfassung

Vom Oktober 2009 bis April 2010 wurde auf der Internistischen Intensivstation der Universitätsklinik Tübingen die neue S3-Leitlinie (LL) zu Schmerz, Sedierung und Delirmanagement als Pilotprojekt implementiert. Im Auftrag der S3-LL- Arbeitsgruppe stellen wir hier das dabei verwendete Konzept vor.


Da unter rein medizinischen Aspekten entwickelte LL inzwischen eine erhebliche Komplexität angenommen haben, muss oft zu deren Umsetzung der Organisationsaufbau und -ablauf einer Station nachhaltig verändert werden. Grundlegende Kennt‑ nisse des Projektmanagements und der Organisationspsychologie sind sehr hilfreich, um solch ein Projekt zum Erfolg zu führen, und werden in diesem Artikel dargestellt. Unser Implementierungsprojekt lief in folgenden Phasen ab: Ziele benennen, Projektteam bilden, Struktur- und Ablaufplan erstellen, Vorher-Messung, Implementierung, Nachher-Messung und Auswertung. Bei der Implementierung kamen folgende Methoden zum Einsatz: Theoretische Schulung über die Ziele des Projekts und über die Inhalte der LL, Audit mit praktischer Schulung am Patientenbett und die Bereitstellung von Informationsmaterial. Zusammenfassend zeigte dieses Projekt, dass praktische Schulungen am Patientenbett die effektivste und effizienteste Maßnahme zur Einführung neuer Arbeitsprozesse ist und andere informative Maßnahmen nur ergänzende Funktion besitzen.

Summary Between October 2009 and April 2010 we implemented the new national S3 guideline for the management of pain, sedation and delirium as a pilot project on our Medical ICU. On behalf of the S3-guideline working group we now present our implementation concept. Owing to the complexity of many new medical guidelines, their implementation often requires significant changes in the organisation and the processes of an ICU. To ensure the success of such a project, a basic knowledge of organi‑ sational psychology and project management is very helpful and this is discussed. Our project comprised the following phases: definition of goals, establishment of a project team, drawing up of a project plan, pre-measurement, implementation, post-measurement, data analysis. For the implementation we used theoretical training covering the goals of the project and the content of the guideline, audits with practical training at the bedside as well as the distribution of information material. Our project showed that the practical work at the bedside is the most efficient and effective method of implementing new processes, while theoretical training and information measures are of only supplementary value.
