Zusammenfassung

Zusammenfassung: Diagnosis-Related Groups (DRGs) führen zur Verkürzung der klinischen Verweildauer der Patienten. Zur optimalen Nutzung der klinischen Ressourcen wurden klinische Behandlungspfade (Clinical Pathways) und standardisierte Behandlungen (Standard Operating Procedures, SOPs) definiert.


Die Anästhesie ist innerhalb der Clinical Pathways ein „Schnittstellenfach“, d.h. die Tätigkeit des Anästhe-sisten tangiert an verschiedenen, voneinander unab-hängigen Abschnitten (oder Modulen) die Tätigkeit anderer Fachdisziplinen: Prämedikationssprechstun-de,die eigentliche Anästhesie,OP-Koordination,post-operative Intensivbehandlung und Schmerztherapie. Die Kosten-Nutzen-Analyse der einzelnen Module muss sich am Endpunkt „Behandlungserfolg“ orientieren. Grundsätzlich wird ein einheitlicher Nutzwert definiert, d.h. das Ziel jeder medizinökonomischen Untersuchung muss der qualitativ hochwertige Behandlungserfolg sein. Qualitativ hochwertige medizinische Versorgung kann durch die Vermeidung von Kosten durch Komplikationen durchaus ökonomisch sein.Der Anästhesist kann kostensenkend zum gesamten Behandlungserfolg beitragen. Kontinuierliche Qualitätsverbesserung (Continuous Quality Improvement, CQI) kann nur durch ein System der Transparenz erreicht werden.Instrumente der kontinuierlichen Qualitätsverbesserung sollten von den beteiligten Berufsgruppen im Gesundheitswesen und der Industrie gemeinsam erarbeitet werden.

Summary Summary: Diagnosis-Related Groups (DRGs) lead to a shorter length of a patient´s stay in hospital.For optimal use of clinical resources, clinical pathways and standard operating procedures (SOPs) were defined. Within the clinical pathways, anaesthesia presents an “interface discipline“,which means that the anaesthe-tists´ performance influences at various, separate stages of treatment (modules) the performance of other specialties, for example regarding premedica-tion, preparation for anaesthesia and informed consent,the anaesthetic procedure itself,coordination of the operating theatre, postoperative intensive care and pain management. A cost-benefit analysis of the various modules has to take into account the overall success of the entire medical treatment.As a basis for assessment, a standard value of benefit has to be defined on the understanding that a successful high quality treatment has to be the objective of any medico-economic analysis. The provision of high quality medical care can,in fact,be economically bene-ficial, as the prevention of complications will also prevent costs.Anaesthetists are therefore able to make their own contribution to the hospital´s economic and therapeutic success. Continuous Quality Improvement (CQI) in health care can only be achieved by a system of sufficient transparency.The task of developing adequate tools of continuous quality improvement should be under-taken jointly by health care professionals and industry.
