Summary Summary: Phenomenologically, oneirism is a waking dream state in which a patient in an extreme trauma- or illnessrelated situation creates an imaginary mental world as a coping mechanism. In contrast to hallucinations, which can occur in response to sensory deprivation, this world is absolutely real for the patient, by whom alone it can be described, i.e. by oral communication, and for whom remains unforgettable. Impressive examples are to be seen in quadroor panplegic polyradiculitis patients whose neurological symptoms are an essential condition for such imaginary experiences. The fundamental aspects of the oneiric state of mind can be discussed on the basis of the experiences of these seriously ill patients as an etiologically non-specific psychopathological syndrome that can be interpreted as a reaction to, and an attempt to cope with, existentially threatening situations.
