Neuromuscular blockade and its monitoring

Zusammenfassung

Zusammenfassung: Eine neuromuskuläre Blockade erleichtert die Intubation und optimiert die Operationsbedingungen. Sie dient nicht der Immobilisation des Patienten zugunsten einer flachen Narkoseführung. Pancuronium eignet sich als lang wirkendes Steroidmuskelrelaxans für Patienten, die nach einem Eingriff für einige Stunden intubiert bleiben sollen.


Nachteilig sind die schlechte Steuerbarkeit des Wirkeffektes und die lange Eliminationshalbwertszeit von etwa 130 Minuten. Vecuronium und Rocuronium sind Steroide mit einer Wirkdauer von etwa 50 bis 60 Minuten. Beide Substanzen werden überwiegend hepatisch eliminiert, ihre Eliminationshalbwertszeiten betragen 70 bis 90 Minuten. Die Besonderheit von Rocuronium ist eine kurze Anschlagszeit von 1 bis 1,5 Minuten. Atracurium und sein Isomer Cisatracurium zerfallen als mittellang wirkende Benzylisochinoline mit einer Halbwertszeit von etwa 20 Minuten überwiegend durch die Hoffmann-Elimination, ein geringerer Anteil wird durch unspezifische Esterasen gespalten. Cisatracurium ist etwa 4 bis 5 mal so potent wie Atracurium. Mivacurium ist ein Benzylisochinolin mit einer Wirkzeit von 20 bis 30 Minuten, das durch die Plasmacholinesterase hydrolysiert wird. Bei Patienten mit atypischer oder verminderter Plasmacholinesterase ist dessen Wirkung verlängert. Histaminliberierende Wirkungen von Atracurium und Mivacurium können nach rascher Injektion (5 s) zu Hautrötungen und einem vorübergehenden Abfallen des arteriellen Blutdruckes führen. Succinylcholin ist das einzige Muskelrelaxans mit extrem kurzer Anschlags- und Wirkzeit. Die typischen Nebenwirkungen, bedingt durch den depolarisierenden Wirkmechanismus, beschränken die Indikation für Succinylcholin auf die Muskelrelaxation während der Narkoseeinleitung des aspirationsgefährdeten Patienten. Inhalationsanästhetika verlängern die Wirkdauer von Muskelrelaxanzien um 30 bis 60%. Einige Antibiotika (z.B. Aminoglykoside) verstärken einen neuromuskulären Block. Bei erheblich eingeschränkter Leber- oder Nierenfunktion sind Atracurium und Cisatracurium aufgrund ihrer organunabhängigen Elimination die Muskelrelaxanzien der Wahl. Zur Aufhebung einer überhängenden Relaxanswirkung wird der Cholinesterasehemmer Neostigmin zusammen mit Atropin verwendet. Grundsätzlich sollte eine neuromuskuläre Blockade erst dann antagonisiert werden, wenn messtechnische oder klinische Zeichen der beginnenden Spontanerholung vorhanden sind. Die Gabe eines Muskelrelaxans ist ein Eingriff in eine Vitalfunktion und bedarf der Überwachung mit einem Nervenstimulator. Ein postoperativer Relaxansüberhang kann zu hypoxischen Komplikationen durch Beeinträchtigung der Schutzreflexe, Verlegung der Atemwege und Minderung der Kraft der Atemmuskulatur führen. Eine fehlende Ermüdung bei der taktilen Erfassung der Double-Burst-Stimulation entspricht einem TOF-Quotienten von etwa 0,8 und gilt als Minimalanforderung an die unmittelbar postoperativ wiedererlangte Muskelkraft.

Summary Summary: Neuromuscular blockade facilitates the placement of an endotracheal tube and establishes optimal surgical conditions. It should not be administered for patient immobilisation during light anaesthesia. Pancuronium is a long-acting steroidal relaxant suitable for patients who have to be ventilated postoperatively for several hours. Disadvantages are the fact that neuromuscular blockade is difficult to regulate, and the long elimination half-life of around 130 min. Vecuronium and rocuronium are steroidal compounds with an intermediate duration of action (DUR90% 50 – 60 min) and an elimination half-life of 70 – 90 min. Both drugs are eliminated primarily by the liver. Rocuronium has a rapid onset of 1-1.5 minutes. Atracurium and its isomer cisatracurium are also intermediate-acting neuromuscular blockers with a half-life of 20 – 25 min that undergo Hofmann elimination and to a smaller extent ester hydrolysis by non-speciÜBERSICHTEN fic esterases. Cisatracurium is about 4 – 5 times as potent as atracurium, and a major pharmacological property is its cardiovascular stability after an intravenous bolus dose. Due to their metabolism the duration of action of both drugs is not prolonged in patients with end stage renal failure or severe hepatobiliary disease. Mivacurium is rapidly hydrolysed by butyrylcholinesterase and has a plasma half-life of 5 min. In patients known to be homozygous for the atypical plasma cholinesterase gene, mivacurium-induced neuromuscular blockade is prolonged by up to several hours. Rapid injection of atracurium at doses higher than 0.5 mg/kg and mivacurium at doses higher than 1.5 mg/kg has been associated with skin flushing and/or transient hypotension. Succinylcholine is the only available muscle relaxant with an extremely rapid onset and an ultrashort duration of action. Because of its side effects its clinical use should be restricted to rapid-sequence induction of anaesthesia in patients with an aspiration risk. Neostigmine reverses residual neuromuscular blockade by increasing the concentration of acetylcholine at the motor end plate by inhibiting acetylcholinesterase. It is advisable not to administer neostigmine until clinical signs or the return of muscle responses after train-of-four stimulation indicate the onset of spontaneous recovery. The depth and duration of the neuromuscular block following a given dose of a muscle relaxant vary from patient to patient. Stimulation of the ulnar nerve and the recording, observation or palpation of the response of the adductor pollicis muscle permits individual adjustment of the muscle relaxant dose in keeping with surgical requirements. This contributes to patient safety by preventing overdosing and ensuring that recovery or reversal of neuromuscular block is adequate. The double-burst-stimulation improves and simplifies the clinical observation of the elicited muscle response, especially during recovery from neuromuscular blockade. Disappearance of fade after a double-burst-stimulation corresponds to a train-of-four-quotient of 0.8 and is considered the minimum requirement for muscle recovery.
