Zusammenfassung: Jedes Krankenhaus muss im Rahmen seiner Dienstaufgaben eine adäquate Notfallversorgung garantieren. Neben rechtlichen Aspekten mit einer umfassenden Sorgfaltspflicht sind es vor allem medizinische Aspekte, die dabei zu beachten sind.


Da bei den Reanimationskenntnissen in der Vergangenheit besonders beim Pflegepersonal teilweise erhebliche Lücken bestanden, bedarf es gerade in diesem Bereich entsprechender Aktivitäten, die sowohl die Aus- wie Fortbildung betreffen. Am Beispiel des Reanimations-Teams am Universitätsklinikum Würzburg wird die Lösung organisatorischer Probleme aufgezeigt. Der Ablauf der Notfallversorgung wird durch eine Notfalldienstordnung geregelt. Der häufigste Einsatzgrund des Reanimations-Teams war mit 28 % ein plötzlicher Kreislaufstillstand, der in 64,7 % erfolgreich behoben werden konnte. Weitere Verbesserungen könnten durch Bereitstellung von Automatisierten Externen Defibrillatoren (AED) auf peripheren Stationen, auf denen 47,4 % aller Akutversorgungen erfolgten, erreicht werden. Neben den organisatorischen Rahmenbedingungen ist vor allem eine adäquate Qualifikation des Personals entscheidend.

Summary Summary: Within the framework of its terms of reference, every hospital has to provide appropriate emergency care for its in-house patients. Apart from legal aspects, medical obligations in particular have to be considered. Since in the past, the practical skills and qualifications of the nursing staff in Basic Cardiac Life Support has clearly proved less than adequate, it is necessary to improve both practical training and education in resuscitation management. We describe how life-threatening medical emergencies are managed at the Würzburg University Hospital. A central emergency call to a constantly available resuscitation team is a prerequisite for structured emergency care. All organizational considerations are detailed in an emergency care policy and procedure manual. The most frequent reason for alerting the resuscitation team was cardiac arrest in 28 % of all the cases. In 64.7 % of these patients resuscitation was primarily successful. Further improvement could be achieved by installing Automated External Defibrillators (AED) on the wards, where 47.4 % of all events requiring the help of the resuscitation team occur. In addition to a structured workflow, improvements in qualifications of all participating staff are mandatory.
