Zusammenfassung: Das hypohysäre Pro­opiomelano­cortin-System wird unter körperlichen sowie emotionalen Stresssituationen aktiviert. Dies führt zur systemischen Freisetzung von Pro­opiomelanocortin-Derivaten: Endorphine, adrenocorticotropes Hormon (ACTH), a-Melanozyten-Stimu­lierendes Hormon und ß-Lipotropin.


Als Aktivatoren des Proopiomelanocortin-Systems konnten körperliche Anstrengung und systemische Ent­zündungs­reaktionen, wie z.B. Sepsis, definiert werden. Die Fragmente des Proopiomelanocortins gelten als Bestandteile eines körpereigenen Stress-Adaptations-Systems, mit dem der Organismus versucht, sich pathophysiologischen Situationen anzupassen. Konsekutiv werden weitere Organfunktionen wie Immunantwort oder Sekretion der Neben­nierenrinde beeinflusst. Trotz zahlreicher Unter­suchungen sind die Regulationsmechanismen des Proopiomelanocortin-Systems bislang unzureichend geklärt. Diese Übersichtsarbeit stellt den gegenwärtigen Wissenstand unter besonderer Berück­sichtigung der Proopiomelanocortin-Derivate ACTH und ß-Endorphin für die Stressadaptation bei körperlicher Belastung und septischen Krankheitsbildern dar.

Summary Summary: The proopiomelanocortin system is activated by different stressors such as physical exercise or systemic inflammation. Pro­opiomelano­cortin serves as a multihormonal precursor for corticotropin (ACTH), lipotropins (ß- and ?-LPH), melanotropins (a-, ß- and ?-MSH) and endorphins. Pro­opiomelanocortin derivatives are considered to be part of the body’s adaptive response to stress. As a consequence, several organ systems, such as im­mune response or adrenocortical secretion can be affected by proopiomelanocortin derivatives. Despite numerous studies to determine the localisation, biosynthesis and regulation of proopiomelanocortin -derived peptides, their function is not completely understood yet. In this review, proopiomelanocortin activation as part of the adaptive response to physical exercise and septic diseases is described. Special consideration has been given to the role of ACTH and endorphins and their relation to stress response and immune function.
