Zusammenfassung
Zusammenfassung: Heparin ist die am häufigsten verwendete Substanz zur Thromboseprophylaxe und -therapie. Bei ungefähr 0,1-5 % der mit Heparin behandelten Patienten entwickelt sich eine immunvermittelte Aktivierung von Thrombozyten und Endothelzellen mit überschießender Thrombinbildung und Thrombozytopenie.
Frühzeitig erkannt und therapiert ist die immunvermittelte HIT gut behandelbar und die Mortalitätsrate von fast 20 % deutlich reduzierbar. Sofortmaßnahmen sind das unmittelbare Absetzen vom Heparin sowie die Einleitung einer Antikoagulation mit einem alternativen Antikoagulans in therapeutischer Dosierung.
In der vorliegenden Zusammenfassung werden die Hauptsymptome und die zu Grunde liegenden Pathomechanismen diskutiert sowie mögliche Therapieoptionen vorgestellt.

Summary: With increasing indications for heparin usage, an understanding of heparin-induced thrombocytopenia (HIT) as a complication of heparin therapy is more essential than ever. In approximately
0.1 - 5 % of patients treated with heparin there is an activation of platelets and endothelial cells as well as the overproduction of thrombin and thrombocytopenia.
Serious sequelae associated with HIT can be avoided with early recognition and appropriate treatment. In this review, we briefly discuss the main features of heparin-induced thrombocytopenia, particularly analyzing the most recent advances in the pathophysiology, diagnosis and treatment of this syndrome.
