Perioperatives medikamentöses Vorgehen bei kardialen Risikopatienten*

Zusammenfassung

Die Ätiologie der perioperativen kardialen Morbidität und Mortalität ist multifaktoriell. Bei der Vielzahl und Verschiedenheit der ätiologischen Faktoren ist nicht zu erwarten, dass eine einzige Maßnahme zuverlässig das perioperative kardiale Outcome nach nicht-kardiochirurgischen Eingriffen verbessern wird.


Deshalb erscheint ein multifaktorielles, schrittweises Vorgehen angebracht. Berücksichtigt man die Pathophysiologie der atherosklerotischen koronaren Herzkrankheit, die nachgewiesene kardioprotektive Wirksamkeit einer inten­siven medikamentösen Therapie im all­ge­mein­­medizini­schen Bereich, das beachtliche Risiko einer koronaren Revaskularisierung bei kardialen Hochrisiko-Patienten, den fehlenden Nachweis einer kardioprotektiven Wirkung einer prophylaktischen präoperativen koronaren Revaskularisierung und das wesentlich erhöhte perioperative Risiko bei Patienten mit Koronarstent, dann kommt der perioperativen intensiven medikamentösen Therapie bei kardialen Risikopatienten eine besondere Bedeutung zu. Die perioperative Stabili­sierung koronarer Plaques mittels aggressiver medi­­kamentöser Therapie mit Statinen, Betablockern, ASS, und ACE-Hemmern ist wahrscheinlich entscheidender als eine Erhöhung der myokardialen Sauerstoffzufuhr mittels koronarer Revaskularisierung. Die aggressive perioperative medikamentöse Behandlung ist sicherlich eine der wichtigsten, wenn nicht sogar die wichtigste perioperative kardioprotektive Maßnahme.

Summary The aetiology of perioperative cardiac morbidity and mortality is multifactorial. In view of the multitude and diversity of aetiologic factors it cannot be expected that a single measure will reliably improve perioperative cardiac outcome following non-cardiac surgery. A multifactorial, stepwise approach would thus appear to be indicated. Taking into account the pathophysiology of atherosclerotic coronary artery disease, the documented cardioprotective effect of intensive medical therapy in the medical population, the considerable risk associated with coronary revascularisation in cardiac high-risk patients, the lack of convincing evidence for a cardioprotective effect of prophylactic preoperative coronary revascularisation, and the considerably increased perioperative risk in patients with coronary stent, intensive perioperative medical treatment of patients at a high cardiac risk acquires a special significance. Perioperative stabilisation of coronary plaques by aggressive medical therapy with statins, betablockers, aspirin and ACE-inhibitors is probably more important than the in­crease in myocardial oxygen supply by coronary revascularisation. Aggressive perioperative medical therapy is certainly one of the most important, if not the most important perioperative cardioprotective measure.
