Zusammenfassung

Die Möglichkeit der Fraktionierung von Vollblut hat in der Transfusionsmedizin zu dem heute fast überall anerkannten Grundsatz der „Hämotherapie nach Maß“ geführt. Bei einigen Indikationen, insbesondere bei grossem Blutverlust durch Traumata, durch Operationen oder postpartal sowie bei mangelnder Verfügbarkeit von EK, FFP, TK und von Gerinnungsfaktoren kann die Transfusion von Vollblut vorteilhaft, sicher und lebensrettend sein.


Zu beachten ist die Gefahr der kardialen Dekompensation infolge Hypervolämie und die erforderliche ABO-Kompatibilität wegen der Isoagglutinine im Plasma. Die Transfusion von Vollblut erfordert eine besondere Organisation und eine intensive Schulung der Ärzte und des medizinischen Personals.

Summary The possibility of fractionation of whole blood in transfusion medicine has led to the now almost everywhere recognized principle of „Haemotherapy to measure“. For some indications, particularly in large blood loss from trauma, surgery, or postpartum, as well as by the lack of availability of RBC, FFP, TK and coagulation factors whole blood transfusion may be beneficial, safe and lifesaving. Note the risk of cardiac decompensation due to hypervolemia and the required ABO compatibility due to the isoagglutinins in plasma. The transfusion of whole blood requires a special organization and intensive training of doctors and medical staff.
