Zusammenfassung

Mit der epidemiologischen Studie EuReCa ONE (European Registry of Cardiac Arrest Study ONE) wurden im Oktober 2014 die Versorgung und das Outcome von Patienten mit einem präklinischen Herz-Kreislauf-Stillstand (OHCA) in 27 europäischen Ländern untersucht.


Die EuReCa ONE Studie zeigte eine hohe Schwankungsbreite in der Inzidenz eines präklinischen Herz-Kreislauf-Stillstandes und im Outcome nach einem solchen Ereignis. Zudem zeigten sich beispielsweise unterschiedliche Interpretationen des Begriffs der Laienreanimation. Basierend auf diesen Erkenntnissen wurde die Nachfolge­studie EuReCa TWO geplant. EuReCa TWO verfolgt drei vorrangige Ziele: 1. Eine Erweiterung des EuReCa- Netzwerks; 2. Ein verbessertes Verständnis über das Rollen-, Alter- und Geschlechtsprofil von Personen, die eine Laienreanimation in Europa durchführen; 3. Eine Abschätzung der Inzidenz und des Outcomes von OHCA in Europa sowohl bei allen Patienten als auch im Hinblick auf spezielle Subgruppen, die auf nationalem Level nur einen kleinen zahlenmäßigen Anteil der Reanimationen darstellen (wie zum Beispiel traumatisch bedingte OHCA). Mit folgenden Maßnahmen sollen die oben genannten Ziele erreicht werden: Unterstützung der teilnehmenden Län­der bei einer landesweiten Da­ten­‑ erfassung Hinzugewinn weiterer Länder als Stu­‑ dienteilnehmer Erweiterung des Studienzeitraums für eine höhere Validität der erhobenen Daten Erfassung von Übereinstimmungen und Unterschieden im Gebrauch des Begriffs der Laienreanimation Erfassung der Inzidenz von Laien­reanimation und deren Einfluss auf das Outcome nach OHCA. Die aktuell aus 29 Ländern beste­‑ hende EuReCa TWO Studiengruppe nutzt einen erweiterten, jedoch weiterhin auf den Utstein-Guidelines aufbauenden Datensatz und ist bei Clinicial Trial.gov unter der Registrierungsnummer NCT03130088 aufgeführt.

Summary EuReCa ONE was a one-month survey of epidemiology, treatment and out­‑ come of patients suffering an out-of-hospital cardiac arrest (OHCA), across 27 European nations, in October 2014. The EuReCa ONE study showed a high degree of variation in incidence and outcome across Europe and raised questions about the cause of these variations. For example, different interpretations of the term `bystander CPR` was noted, confirmed by a subsequent European survey. The need to further understand the cause of variation led to the establishment of the EuReCa TWO project. EuReCa TWO has three primary aims: 1) Expand the EuReCa network; 2) Improve the understanding of the role, age and gender profile of bystanders in OHCA in Europe; 3) Generate estimates of European OHCA incidence and out­‑ come for all patients as well as subgroups that make up a small proportion of overall cases at national level, e.g. traumatic aetiology, patients transferred to hospital with ongoing CPR. In order to achieve these aims, the following objectives will be fulfilled: encourage participating countries to aim for national data collection and encourage additional countries to participate in the study; increase the data collection period in order to provide more robust estimates of incidence, management and outcome; identify consistency and variation in the use of the term ‘bystander CPR’; describe the incidence of ‘bystander CPR’ and its influence on OHCA outcome. With 29 countries now part of the EuReCa ‘family’, the EuReCa TWO study provides an expanded Utstein-based dataset, data collection tool over a three-month data collection period for all participating countries. The EuReCa TWO study is registered by ClinicalTrials.gov (ClinicalTrials.gov, Re­gistration Number: NCT03130088).
