Die Anwendungspraxis von Metamizol und Paracetamol bei der antipyretischen Therapie auf Intensivstationen deutscher Universitäts­kliniken – eine web­basierte Umfrage

Zusammenfassung

Hintergrund: Metamizol und Paracetamol werden in der perioperativen Medizin häufig zur Analgesie und Antipyrese verwendet. Insbesondere im intensivmedizinischen Kontext fehlen derzeit Daten zu Nutzen und Risiken für eine medikamentöse antipyretische Therapie mit Metamizol und Paracetamol.


Vor diesem Hintergrund ist auch die Praxis der Anwendung dieser Substanzen hinsichtlich der Indikations- und Kontraindikationsstellung auf Intensivstationen unklar. Gegenstand der vorliegenden Arbeit war daher eine Evaluation der derzeitigen Praxis beim Einsatz von Metamizol und Paracetamol zur antipyretischen Therapie hinsichtlich ihrer Indikationen und Kontraindikationen auf Intensivstationen deutscher Universitätskliniken. Studiendesign und Untersuchungsmethoden: Die Umfrage erfolgte online mittels Fragebogen im Zeitraum von Februar bis August 2016. Dazu wurden ärztliche Kollegen unterschiedlicher Fachdisziplinen in deutschen Universi­tätskliniken angeschrieben, die an der intensivmedizinischen Versorgung teil­nehmen. Ergebnisse: Insgesamt lagen 344 voll­ständig ausgefüllte Fragebögen zur Aus­wertung vor. Nahezu alle Kollegen führen eine medikamentöse antipyretische oder eine analgetische Behandlung mit Metamizol oder Paracetamol durch. Der Temperaturgrenzwert zur Indikationsstellung schwankt dabei zwischen 37,5º und 40º Celsius. Die Datenlage zu Indikationen und Kontraindikationen zeigt ein heterogenes Bild und steht teilweise im Widerspruch zu den jeweiligen Fachinformationen. Schlussfolgerung: Metamizol und Pa­racetamol werden auf deutschen In­tensivstationen häufig eingesetzt. Die heterogene Indikationsstellung reflektiert dabei unseres Erachtens die lückenhafte Datenlage zu Nutzen und Risiken dieser Substanzen. Angesichts der regelhaften Verwendung im intensivmedizinischen Kontext halten wir klinische Studien zur Verwendung dieser Substanzen für dringend erforderlich.

Summary Background: Acetaminophen (paracetamol) and dipyrone (metamizole) are regularly used in perioperative medicine. The evidence regarding their risk/benefit ratio especially in the critical care environment is insufficient. Accordingly, the relevance of the potential side effects of dipyrone and acetaminophen in critically ill patients is unclear. Little is also known about the practical aspects of their use in the critical care environment with regard to their indications and contraindications. Thus, the aim of this study was to investigate the indications and contraindications for the use of dipyrone and acetaminophen in German intensive care units. Method: We conducted this survey from July to August 2016 using an online questionnaire. We sent this questionnaire to physicians who work in intensive care units at 16 German university hospitals. Results: 344 questionnaires were fully answered and subsequently analysed. Our data show that most physicians use dipyrone or paracetamol to treat fever and pain in the critical care environment. As far as the the indication for treatment with these medications was concerned, we found that the threshold for fever treatment varies between 37.5° C and 40°C. We also found a profound deviations from the contraindications stated in the respective SPCs for treatment with dipyrone and paracetamol. Conclusions: Taken together, dipyrone and paracetamol are regularly used for the treatment of fever and pain in the intensive care units in our questioned collective. However, there is a remark­able variance with regard to indications and contraindications. We interpret this variance as sign for the lack of treatment data in the critical care environment of these medications. Accordingly, we think further studies with a focus on the use of these substances are urgently needed.
