Hohe Spinalanästhesie nach ambulanter epiduraler Injektion von Ropivacain zur Schmerztherapie

Zusammenfassung

Nach ambulanter epiduraler Injektion von Ropivacain und Triamcinolon zur Schmerztherapie kommt es bei einem 75-jährigen Patienten zu Kollaps und Somnolenz. Im Verlauf fällt eine Schwäche beider Beine auf. Der Notarzt bringt den Patienten unter der Verdachtsdia-gnose „neurologisches Geschehen DD Anaphylaxie“ in die Notaufnahme. Nach Notfalluntersuchung wird die Diagnose einer akzidentellen hohen Spinalanästhesie gestellt, die weitere Behandlung erfolgt auf der Intermediate Care-Station.

Summary

After epidural injection of ropivacaine and triamcinolone in an outpatient setting, a 75-year-old patient suffered  from collapse and somnolence. In addition, a weakness in the lower limbs on both sides appeared. The EMS was called, transferring the patient to the ED with suspected “neurological deficit dd anaphylactic reaction”. After emergency evaluation the patient was transferred to the Intermediate Care Unit with diagnosed accidental spinal anaesthesia.
