Intraoperative, maschinelle Autotransfusion bei bakteriell kontaminiertem Operationsgebiet – Eine In–vitro-Studie

Zusammenfassung: Die maschinelle Autotransfusion ist ein etabliertes Verfahren in der orthopädischen, abdominalen und Kardio-Chirurgie. Aber sie ist bisher noch nicht zugelassen für die Anwendung in einem bakteriell kontaminierten Operationsgebiet, z.B. der Mundhöhle. 2003 und 2004 haben wir an der Klinik für MKGChirurgie, plastisch-ästhetische Operationen Krefeld, eine klinisch orientierte In-Vitro-Studie durchgeführt.


Dabei wurde bei 45 ausgedehnten Dysgnathiechirurgischen Eingriffen das mit Speichel und Bakterien der Mundhöhle kontaminierte Blut gesammelt. Mit Hilfe der Waschzentrifugation im CATS®, einer Leukozytendepletion, einer antimikrobiellen medikamentösen Behandlung sowie einer selektiven Endotoxinadsorption wurde das gesammelte Wundblut aufbereitet, gereinigt und zu einem autogenen Erythrozytenkonzentrat aufkonzentriert. Zur Untersuchung der Qualität wurden hämatologische und gerinnungsphysiologische, immulogische und mikrobiologische Parameter analysiert. Zusätzlich wurde die Endotoxinkonzentration vor, während und am Ende der Aufbereitungsprozedur bestimmt. Die Reihenfolge der einzelnen Reinigungsschritte und die Häufigkeit ihrer Verwendung wurden mehrfach geändert, bis die effektivste Sequenz gefunden war. Der Einsatz des Cell-Saver sowie die Leukozytendepletion konnten einen beträchtlichen Anteil der aufgesammelten Keime der Mundhöhle eliminieren. Allerdings nur durch die extrakorporale Zugabe von Antibiotika und eines Antimykotikums war eine reproduzierbare Bakterienelimination unterhalb der Nachweisgrenze möglich. Endotoxine konnten bis auf 99,9% ± 0,1% durch die einzelnen Aufbereitungsschritte einschließlich des spezifischen Endotoxinadsorbers reduziert werden. Die Hämolyserate betrug am Ende 0,2% ± 0,1%. Zytokine und Fibrinspaltprodukte wurden zu mehr als 90% eliminiert. Die zugesetzten antimikrobiellen Substanzen wurden zwischen 95,8 % ± 11,7% und 92,0% ± 15,4 % wieder eliminiert. Aufgrund der mehrstufigen Aufbereitungsprozedur hätte ein intraoperativer Blutsverlust von ungefähr 50% kompensiert werden können. Durch die Kombination aus Cell-Saver, Leukozytendepletion, antimikrobieller medikamentöser Behandlung und einer selektiven Endotoxinadsorption konnte ein autogenes Erythrozytenkonzentrat aus einem bakteriell kontaminierten Operationsgebiet reproduzierbar keimfrei hergestellt werden. Endotoxine konnten bis auf einen absoluten Gehalt von 1 ng ± 1 ng eliminiert werden.

Summary Summary: Intraoperative autotransfusion is an established procedure in orthopaedic, abdominal and cardio-surgery, which, however, is not yet approved for use in bacterially contaminated surgical sites, e.g. the oral cavity. In 2003 and 2004 at the hospital for MKG surgery, plastic-cosmetic operations in Krefeld we carried out a clinically oriented in-vitro study. During 45 extensive dysgnathia operations blood contaminated with bacteria and spittle from the oral cavity was collected. With the aid of the centrifugal washing facility in CATS® leukocyte-depletion, antimicrobial medication and selective endotoxin adsorption, the collected blood was processed, and concentrated erythrocytes obtained. To test for quality, haematological, coagulation-physiological, immunological and microbiological parameters were analysed. Furthermore, the endotoxin concentration was determined prior to, during and after processing. The sequence of the individual processing steps and the frequency of their use was altered repeatedly until the most effective sequence was established. Through the use of the cell saver, together with leukocyte- depletion, a substantial proportion of the pathogens collected from the oral cavity were eliminated. However, only through the extracorporeal addition of antibiotics and an antimycotic agent did it prove possible to reproducibly eliminate bacteria to below the detection limit. As a result of the various processing steps and the use of the endotoxin adsorber, endotoxins were reduced by as much as 99.9% ± 0.1%. The final haemolysis rate was 0.2% ± 0.1%. More than 90% of the cytokines and fibrin degradation products were eliminated. Between 95.8 % ± 11.7 % and 92.0 % ± 15.4 % of the added antimicrobial substances were eliminated again. Using the multistep processing procedure, an intra-operative blood loss of approximately 50% could have been compensated. Through the combined use of cell saver, leukocyte-depletion, antimicrobial measures and selective endotoxin adsorption, a germfree autogenous erythrocyte concentrate was reproducibly obtained from a contaminated surgical site; endotoxins were eliminated to an absolute concentration of 1 ng±1ng.
