Zusammenfassung
Zusammenfassung: Die erfolgreiche Behandlung von Koagulopathien erfordert eine rasche Gerinnungsanalytik. Die klassischen Gerinnungsbefunde des Zentrallabors, Prothombinzeit (PZ) und aktivierte partielle Thromboplastinzeit (aPTT) sind zum einen zeitaufwendige Prozesse, zum anderen sind die Ergebnisse wenig prädikitv, was die Vorhersagekraft von Blutungsneigungen und Transfusionsbedarf betrifft.
Im Gegensatz dazu stellt die Thrombelastometrie/graphie ein „point of care“-taugliches Analyseverfahren dar, das nicht nur die Initiierung der Gerinnung, sondern auch die Gerinnselbildungsgeschwindigkeit und den Clot als Gerinnungsprodukt erfasst. Für die Erkennung einer Hyperfibrinolyse stellt die Thrombelastographie/metrie den Goldstandard dar. Die Erfassung dieser Parameter erfolgt als graphische Darstellung.
Summary
Summary: Rapid monitoring of coagulation is essential for the treatment of patients with coagulopathy to be successful. The common coagulation parameters, prothrombin time und activated partial thromboplastin time are time-consuming processes and these measurements are hardly predictive of a bleeding tendency and transfusion requirements. In contrast, thrombelastometry/graphy is a promising point-of-care monitoring device which not only shows the initiation of coagulation, but also the speed of clot formation and maximum clot firmness. Thrombelastometry/ graphy is considered the gold standard to detect hyperfibrinolysis. All these measurements are recorded as a graphic representation of clot formation and subsequent lysis.
